TERMS AND CONDITIONS

Total participated employees: min. 5 persons and max. 50 persons

Age of employee and spouse: min. 17 years and max. 59 years;

Child age with min. 0 day and max. 23 years (student, unemployed, unmarried
fully financial depending upon the Employee);

All employees must be enrolled into the Plan;

Employees with the same position must be enrolled into the same Plan;

Different Plan for each position can be done up to 2 (two) plan levels;

Plan should be determined at the beginning of insurance. Additional Plan is not allowed
if the insurance has started;

Determination of Out-patient & Dental Benefits Plan may be different from the Inpatient Benefit
Plan to the extent it will not more than 1 (one) plan below;

Attaching the supporting documents as required;

PROTECTION COMPREHENSIVE HEALTH SOLUTION

Exclusions* :
For Inpatient, Outpatient, Surgical and Dental Treatment Benefits:
 Pre-existing conditions whereby the Member has received medical treatment, diagnostic, or consultation whether in a conscious or
unconscious condition.
 Becoming a Member within 90 (ninety) days before the effective date of membership unless he/she is insured under this Policy for 12
(twelve) consecutive months since the effective date of membership.
 Pre-existing disorder or disease and the complication thereof.
 All conditions related to pregnancy/birth that require treatment/surgical procedure, fertility treatment.
 Any costs or tariffs exceeding the permitted maximum benefit for the use of room and other additions for personal comfort.
 Aesthetic treatment/medication or cosmetic/plastic surgery, including weight-loss medicine.
 Or addictive disorder caused by the use of illegal drugs or alcohol addiction, mental illness, suicide attempt, veneral disease and the
consequence thereof.
 Eye refraction, any Inpatient treatment related to the dental problem and examination/routine medical check-up.
 Treatment as a direct or indirect result of war or riot or mass violence, strike, military duty, or crime, as a passenger of a chartered flight
or a dangerous sport.
 Alternative treatment (Sinshe and the like), treatment for health recovery.

For Life Insurance Benefit:


 Suicide within the first 12 (twelve) months of the membership.
 Pre-existing conditions whereby the Member has received medical treatment, diagnostic, or consultation whether in a conscious or
unconscious condition.
 Becoming a Member within 90 (ninety) days before the effective date of membership unless he/she is insured under this Policy for 12
(twelve) consecutive months since the effective date of membership.

For Accidental Insurance Benefit:


 Participating in a risky competition or activity, among others: parachuting, ice skiing, water skiing, martial art, mountain climbing.
 Direct or indirect involvement in war, rebellion, riot, coup d’etat.
 Suicide, working at an airline company, using illegal drugs and consequence of atom/nuclear reaction.
 Participating in military duty or military operation.
