The long-bone fractures occur most frequently in the
tibial shaft. Adequate treatment of such fractures avoids
consolidation failure, skewed consolidation and reoperation.
To classify these fractures, the AO/OTA classification
method is still used, but it is worthwhile getting to know the
Ellis classification method, which also includes assessment
of soft-tissue injuries. There is often an association with
compartmental syndrome, and early diagnosis can be
achieved through evaluating clinical parameters and
constant clinical monitoring. Once the diagnosis has been
made, fasciotomy should be performed. It is always difficult
to assess consolidation, but the RUST method may help in
this. Radiography is assessed in two projections, and points
are scored for the presence of the fracture line and a visible
bone callus. Today, the dogma of six hours for cleaning
the exposed fracture is under discussion. It is considered
that an early start to intravenous antibiotic therapy and the
lesion severity are very important. The question of early or
late closure of the lesion in an exposed fracture has gone
through several phases: sometimes early closure has been
indicated and sometimes late closure. Currently, whenever
possible, early closure of the lesion is recommended,
since this diminishes the risk of infection. Milling of the
canal when the intramedullary nail is introduced is still a
controversial subject. Despite strong personal positions in
favor of milling, studies have shown that there may be some
advantage in relation to closed fractures, but not in exposed

A clinical trial by Kindsfater et al(18) supports the
principle of treatment within six hours. The authors,
when analyzing the treatment of 47 patients with open
tibial fracture, observed a greater incidence of infection
in patients treated after more than five hours.
One of the first studies to raise questions about the
rule of treatment within six hours was that of Patzakis
and Wilkins(19), who found infection rates of 6.8% in
injuries treated within 12 hours and 7.1% in those
treated after 12 hours, with a difference that was not
statistically significant.
Harley et al(20) carried out a retrospective review of
215 cases of open fractures to assess the correlation
between time-to-treatment and delayed consolidation
and infection rate. Although 46% of patients were
treated more than eight hours after the accident, it
was not possible to establish a correlation between
the delayed treatment time and the complications. It
was clearly established, however, that their finding
on increased infection rate was directly proportional
to the severity of the injury.
In another study, Spencer et al(21) evaluated 142
open fractures, 60% of which were treated within the
first six hours following the accident. The overall rate
of infection was 10.4%, and it was not possible to
establish any statistical significance between the cases
treated within six hours or after this period.
As it can be observed, the studies fail to demonstrate
the truth of the six-hour rule; however, all of them
show sufficient evidence of the relationship between
infection and severity of the injury. Two other facts to
be taken into consideration are the conclusion drawn
by Patzakis and Wilkins(19) who claim that “the most
important factor in reducing the infection is early

and Anderson(22) who claim that debridement is the
most important factor to achieving a good outcome
for open fractures. The most appropriate approach
would probably be somewhere between these two
ideas – antibiotics should be administered as early
as possible, and good lavage and debridement
should be performed.
In the review by Crowley et al(23) on time and infection,
the authors concluded that the principle of
treatment within six hours must be revised, but debridement
should be performed as early as possible.
Open tibial fracture – primary or delayed closure?
Several phases have been assigned to the treatment
of fractures after cleaning, with primary or delayed
closure. For a long time, delayed closure of the injury
was recommended, as in the American experience in
the World War II and the Vietnam War, when infection
rates for delayed closure injuries were 2.5%(24). The
classic study by Gustilo and Anderson(22) on early
closure of grade I and II injuries showed an infection
rate of 6%, but the closure of grade III injuries led

to rates of 44%

Suatu hari, aku melamun pikiranku melayang. “Sudah banyak aku melakukan lomba, tapi kebanyakan
tidak menang.” Itulah pikiranku.
Tiba-tiba Ustadzah Haria memanggil. Ternyata aku terpilih untuk menjadi wakil sekolah menjadi
salah satu peserta lomba puisi. Pada hari ini juga aku mulai berlatih untuk lomba itu. Lomba akan
dilaksanakan 1 minggu lagi.
Tak terasa satu minggu telah berlalu. Hari ini adalah hari lomba. Selain aku, temanku yang ikut adalah
Nayfa dan Farih. Tapi mereka beda bidang lomba denganku. Nayfa MTQ sedangkan Farih kriya
anyam.

Sekarang aku sudah mulai menulis puisiku. Ini sudah mulai lomba lho. Setelah selesai puisiku pun
dinilai. Diambil lima besar. Alhamdulillah aku masuk lima besar. Lima besar akan membacakan
puisinya. Aku deg degan saat maju membacakan puisiku.
Karena lomba sudah selesai, aku pun pulang. Pengumuman pemenang akan diumumkan pada hari
Selasa.

Hari ini adalah hari pengumuman lomba. Alhamdulillah aku mendapat juara tiga. Nayfa juara satu
dan Farih juara tiga. Tak apalah juara tiga. Dan itulah pengalamanku.

Tawa mereka masih juga terdengar di telingaku, aku tau semuanya telah berbeda. Kumohon sobat!!!
Lihatlah aku, lihatlah keberadaanku. Tapi entah mengapa satu kali pun kalian tak melihat
keberadaanku, tak melihat rasa sayangku kepada kalian. Kenapa sobat! Kalian justru menganggapku
sebagai sesosok wanita yang tak berguna bahkan untuk sekedar menyapaku saja kalian tak mau. Lalu
apakah aku harus memohon kepada kalian jika aku ingin diperhatiakan oleh kalian.

Pagi ini suasana tak jauh berbeda, semuanya asik dengan kesibukan mereka, begitupun denganku.
Aku tau mungkin ini terakhir kalianya aku melihat senyuman itu, sobat, aku sayang pada kalian
semua, aku tak ingin hari ini berlalu begitu saja, andai kalian tau bahwa hari ini adalah hari dimana
diriku harus berpisah. Aku tak ingin melewatkannya teman, sungguh!

“Kamu kenapa?” Tanya sasa yang terlihat merasa ada yang aneh dengan diriku hari ini.
“Aku hanya ingin melihat senyuman kalian sebelum aku pergi.”

“Lupakan saja. Sudahlah sana main lupakan aku untuk sejenak saja, dan saat itu kamu akan mengerti
posisiku sekarang. Saat dimana keramaian menjadi sebuah keheningan untukku saja. Aku lelah untuk
berdiri seorang diri, jangankan untuk berdiri tertawa pun aku tak memperlihatkannya, kamu tau,
seandainya kamu dan mereka semua tau. Tapi, nyatanya tak ada satu orang pun yang memahaminya,
lalu apakah dayaku sa! Oh ya jika aku terlalu lama tidur berarti aku lelah dan ingin istirahat, jadi
jangan bangunkan aku ya sa!”

“Aku tak menyangka bahwa itu terakhir kalinya aku melihat senyuman diana. Terkadang aku sempat
berpikir kenapa aku justru tak pernah menyadari bahwa diana adalah satu-satuanya orang yang sangat
ingin melihat kita bahagia bahkan sampai berkorban dengan mengaku sebagai kau hilmi, kau lah
alasannya kenapa kita semua disuruhnya untuk pulang waktu itu.” semuanya diam dan tertegun
mendengarnya.
“Karena dia tak ingin kalian melihat itulah terakhir kalinya dia bernafas, dia rela mengatakan pada
mereka orang yang sengaja ingin membuhuhmu hil jika dia adalah kamu.”

Semuanya terdiam seribu bahasa menyesali perbuatan mereka yang tak menganggap orang yang
hanya memperhatikannya akan melakukan ini semua, melakukan hal yang tak pernah mereka duga.
