A Base-Supported Tube and Coupler Scaffold: This must be constructed with 6-foot
(1.8 m) post spacing, 3 lifts high, 4 bays long, 5 planks wide, and tied at 2 points.
The following faults must be placed on the scaffold;
• Three adjacent posts jointed in the same bay/ lift
• Two runners (ledgers) joined in the same bay
• Single couplings installed on the box ties
• Intermediate bearers (boards bearers) installed at 24” (600 mm)(2 feet) from the end
of the planks on the butted joints
• A mid rail missing

A Under Hung Scaffold: This must be 10 feet (3 m) long, and a minimum of 4 boards
wide.

The following faults must be placed on the scaffold;


• No safety couplers at the top of the hanger tubes
• Guardrails installed outside the hanger tubes
• Boards with gaps in excess of 1” (25 mm)
• A sloping ladder not secured at the top

ALTERNATE No.2

A Base-Supported Tube and Coupler Scaffold: This must be constructed with 6-foot
(1.8 m) post spacing, 3 lifts high, 4 bays long, 5 planks wide, and tied at 2 points.

The following faults must be placed on the scaffold;


• The posts (standards) not plumb on sleeve joints
• Bracing missing on back side
• The inside cross member of the box tie not installed
• Toe boards missing
• A ladder installed with the ratio incorrect

The Under Hung Scaffold: This must be 10 feet (3 m) long, and a minimum of 4 boards
wide

The following faults must be placed on the scaffold;


•The supporting cross tubes not secured correctly
•The safety couplers missing
• The intermediate transoms (board bearers) not installed
• No access ladder
ALTERNATE No.3

A Base-Supported Tube and Coupler Scaffold: This must be constructed on soft


foundations, 6-foot (1.8 m) post spacing, 2 lifts high, 4 bays long, 5 planks wide, and tied
at 2 points.
The following faults must be placed on the scaffold;
• Missing base plates
• The runners (ledgers) jointed with sleeve couplers at 18” (1.5 feet) from the posts
(standards)
• Bracing not installed on top lift
• The planks installed at only 2” (50 mm) overlap on the end bearers (transoms) ·
A ladder installed with no ties at the top

The Under Hung Scaffold: This must be 10 feet (3 m) long, and a minimum of 4 boards
wide

The following faults must be placed on the scaffold;


• Some runners (ledgers) installed on the outside of posts (standards)
• The end bearers (transoms) installed post (standard) to post (standard) on the
working platform
• The trapeze tubes secured with swivel (round) couplers
• The guardrails installed at 30” (760 mm) above working platform

ALTERNATE No.4

A Tube and Coupler Mobile Scaffold: This must be constructed with a working height
of 14 feet (4.25 m), and 5 planks wide.

The following faults must be placed on the scaffold;


• Defective/ missing brakes on the castor wheels
• Swivel (round) couplers installed on the post (standard)/ bearer (transom)
• Missing plan bracing
• Guardrails installed on the outside of posts (standards)

The Under Hung Scaffold: This must be 10 feet (3 m) long, and a minimum of 4 boards
wide.

The following faults must be placed on the scaffold;


• Single clamps installed to connect supporting cross tubes
• No safety couplers on the underside of trapeze tubes
• Excessive overhang on the boards
• No access

ALTERNATE No.5
A Tube and Coupler Tower Scaffold: This must be constructed with a base of 5’.6”
(1.7 m) × 4 feet (1.2 m), and 3 lift high.

The following faults must be placed on the scaffold;


• A kicker/ base installed to an excessive height
• The bearers (transoms) installed on the underside of runners (ledgers)
• The top plan bracing missing
• Transverse bracing installed post (standard) of runner (ledger)
• Missing mid-rails

The Under Hung Scaffold: This must be 10 feet (3 m) long, and a minimum of 4 boards
wide.

The following faults must be placed on the scaffold;


• A missing intermediate transoms (board bearer)
• A missing guardrail
• The planks overhanging the end bearers (transoms) by less 6” (150 mm) or more
than 12” (300 mm)
• No access
• Missing check (safety) couplers

ALTERNATE No.6

A Light-Duty, Tube and Coupler Mobile Scaffold: This must be constructed with a
base of 5’.6” (1.7 m) × 4 feet (1.2 m), and 3 lift high.

• Damaged castor wheels (Defective rims-difficult to move mobile scaffold)
• The bottom plan brace missing
• The bracing more than 12” (300 mm) from node point
