In FMA 1, you interviewed an outstanding teacher in your community on her/his personal

experiences as a teacher as well as views on certain issues related to teaching. For FMA 2,
you will conduct a mini-survey in your community of at least 12 community members as
respondents, specifically, three (3) students, three (3) parents, three (3) LGU
officials/community leaders and three (3) teachers. Make sure that each interviewee does
not have overlapping roles e.g. as parent and teacher; teacher and community leader.

Instructions:

1. Prepare an open-ended survey questionnaire* asking their views/opinions on

priorities, importance of some issues on the current Philippine education system:

You may want to adapt the attached questionnaire and translate the questions and
options to the language the respondents are most familiar with.

2. Write a report on the survey results. The last paragraph should be on what you
learned and realized after doing the survey and writing the report.

3. Attach the following to the report:

 Photos showing you fielding and retrieving the survey forms
