2. Request that CCL, ACG conduct digital forensic examination on the accompanying
specimen specifically describe on the attached request form.

3. Background of the case with the following information:

a) NATURE OF CASE : Cybercrime – Blackmail

b) VICTIM/S : Maria De La Cruz, 33 years old, resident of number

105, Lamtang, Irisan, Baguio City

c) SUSPECT/S : Jose Manglisan

d) T D P O : On or about 8:00 a.m. on July 02,2019 at Fifth floor

( Research and Defense room), U.C.- CCJE
Department, Legarda road, Baguio City
4. Facts of the Case:
On or about 8:00 o'clock in the morning on July 02, 2019, an alleged crime
was committed at the fifth floor(defense and research room) of UC-CCJE Department,
Legarda road, Baguio City. The suspect, Jose Manglisan, 21 years old, an Information
Technology of University of the Cordillera, Governers pack road, Baguio City was about
to blackmail his instructor, Maria De La Cruz, 33 years old, Information Technology
instructor of University of the Cordillera and a resident at number 105 Lamtang, Irisan,
Baguio City, by uploading their pornographic video online which is saved in his flashdrive
if she will not pass him in his subject.

After copying the video, the suspect then hid the flashdrive in a styrofoam for
him to conceal it. He then immediately vacate the crimescene.
