Assurance and Billing, duly representing IP Converge Data Services Inc., with business
address at 34F RCBC Plaza Tower II, 6819 Ayala Avenue, Makati City do hereby
voluntary depose and say;

That the company is engaged in Other Service Activities and existing under IP
Converge Data Services, Inc;

That the company would like to apply for an updating of their Authority to Use
Loose leaf Permit for Continuous Receipts using specifically as follows:

a. Billing Statement - SN: 0047501-9999999

That the above mentioned commit to permanently bind the loose-leaf forms within
(15) days after the end of each taxable year or upon the termination of its use.

That in the event that the foregoing be discovered to be in violation of existing

rules and regulations, I hereby undertake to face any legal sanctions and pay
corresponding penalties thereof as provided for under the National Internal Revenue Code
of 1997;

I HEREBY DECLARE UNDER PENALTIES OF PERJURY THAT THE
