Purpose of the Assignment The purpose of the assignment is to provide the student the opportunity to summarize in detail the development, implementation, and evaluation of an instructional intervention for a specific patient or professional group. Approach to Assignment I approached this assignment with the intent to perform community education because I feel there is always a need to educate the community on health promotion, disease prevention and risk reduction. I decided to focus on Diabetes because of its growing epidemic in the United States and my interest in becoming a Certified Diabetes Educator. Reason for Inclusion This paper exemplifies my creativity in identifying and executing a plan to address the educational needs of a community. Critical Thinking Revises actions and goals based on evidence rather than conjecture. In this paper, I had to revise my goals of the teaching project after completing the needs assessment. Initially, I thought my focus was going to be about diabetes management but then I soon realized that none of the learners had a diagnosis of diabetes nor had they been tested but they all had risk factors of Type 2 Diabetes. Due to this evidence, I revised my actions and goals of the teaching project to reflect the identified needs of the group. Engages in creative problem solving. At the last minute, my scheduled date and time to execute the teaching project had been cancelled so I had to think quickly to come up with a resolution to this problem. I ended up having to change the date, time and location of the teaching project. When doing this, I had to make sure that the learners would be available to attend considering it was outside of normal scheduled meeting times. Nursing Practice Applies appropriate knowledge of major health problems to guide nursing practice. In this paper, I utilized my knowledge of Diabetes to help guide my nursing practice. “More than 30 million people in the United States have diabetes and one fourth of them don’t know they have it (“Diabetes Quick Facts,” n.d.)” To address this problem, I decided to perform an educational session about the prevention and risk reduction of Type 2 Diabetes. Establishes and/or utilizes outcome measures to evaluate the effectiveness of care. I used content evaluation and recall to evaluate the effectiveness of my teaching session. I quizzed the readers to see how well they comprehended the information being taught. I asked the learners to state the signs and symptoms and risk factors of Type 2 Diabetes and write down a list of their personal risk factors and three ways to decrease those risk factors. All readers were able to achieve these tasks which proved my teaching session was effective. Communication Accesses and utilizes data and information from a wide range of sources to enhance patient and professional communication. I used a variety of sources to complete my teaching project to enhance patient and professional communication. I used information from the Centers for Disease Control, American Diabetes Association, American Association of Diabetes Educators, the course textbook and pamphlets. Expanding my knowledge using these sources enhanced my communication during the teaching session. Teaching Provides relevant and sensitive health information and counseling to patients and families. During this teaching project, I provided information regarding the prevalence and epidemic of Diabetes. I also educated the learners on the signs and symptoms, risk factors and ways to prevent their risk of obtaining Diabetes. Uses information technologies and other appropriate methods to enhance one’s own knowledge based Using information technologies such as websites and journals to understand the entire disease process of Type 2 Diabetes helped enhance my base knowledge so that I could share this knowledge with the learners of the group. Uses information technologies to communicate health promotion, risk reduction and disease prevention. During this teaching project, I used lecturing and handouts to talk about the health promotion, risk reduction and disease prevention of Type 2 Diabetes. Leadership Assumes a leadership role within one’s scope of practice as a designer, manager and coordinator of health care to meet the needs of populations. During this teaching project, I took the leadership role to design a plan, manage and coordinate the execution of teaching plan to meet the health care needs of the low-income population of the Fast Forward program. I was able to determine the needs of the community through the needs assessment then develop and implement a plan to address those community needs.
