Certificate of Live Birth and Certificate of Death;

[3] However, her name was entered as NEOLINA AXALAN

DEL MUNDO in her records in the One Puhunan Microfinance;

[4] As two disinterested persons, we attest that the name

NEOLINA BAY AXALAN appearing in the said Certificate of Live
Birth and Certificate of Death and the name NEOLINA AXALAN DEL
MUNDO appearing in the mentioned records of One Puhunan
Microfinance, refer to one and the same person;

[5] We are not related to NEOLINA BAY AXALAN by

consanguinity or affinity;

[6] We are executing this affidavit to attest to the truth of all

the foregoing facts and circumstances and for whatever legal purpose
this may serve.
