or before August 17, 2017 (Thursday) the following, all signed by the Team
Manager:

• Official primary and secondary color for the dark uniform

• Official team name
• Official list of bench personnel
• Completely filled out Bench Personnel Forms with ID picture (blank forms
attached)
• Official list of beneficiary/ies, with details such as the official name of the
foundation, purpose, contact person and contact number

With the very tight timeline going to the Opening of the Season, we require the timely
submission of these enumerated requirements.
