around treatment area in patients room. communicates well with nursing pre and post PT session to gather or
provide info and updates. monitor patient's vitals as indicated. collaborate with other rehab discipline if co-
treatment is indicated. Maintains universal precaution at all times. Requires cues and supervision 75 % of the
time during Evaluation and treatment session. Gives clear instruction to patient and what to anticipate during
treatment session. Ashley maintains good body mechanics that is safe for her and her patient.

CI Final — Ruth G. Mashak Ashley has become more aware of the physiological changes and signs and symptoms to look for to adjust
proceed or modify treatment plan. Maintains a safe working environment and prepares equipment needed for

nursing staff pre post treatment. Request assistance when necessary. Addendum: Ashley was graded as 8
advanced Beginner on her midterm grade but due to computer glitch she was mistakenly graded as beyond entry
level. Ashley has shown significant improvement since her midterm to current status.

HPI that is relevant to the case. She conducts patient interview in a smooth and organize manner. Exhibits
competence in taking pt. vitals signs, and monitors them accordingly during treatment session. She is proficient
in testing level of pt. arousal and cognitive level, LE ROM, mm strength testing, balance testing , transfer and
bed mobility assessment, gait assessment and motor function.

CI Final — Ruth G. Mashak Ashley is teachable and listens to feedbacks without defensiveness. Very interested in learning and highly
motivated to excel. Very passionate and advocates for her patients. Communicates well verbally and
nonverbally and good in reading body language from her patient and responds accordingly. She knows how to
establish rapport with her patients and they appreciate her well. Ashley is ready to tackle the Rehab world and
with greater experience she will only do well and will be an asset in this profession.

CI Final — Ruth G. Mashak Ashley has good instinct which is to her advantage. She needs more experience in Neuro and cardiopulmo cases
so She can be more confident when dealing with such cases with multiple comorbidities.

CI Mid-Term — Ruth G. Mashak 10 hour day schedule a full caseload for a new graduate is 15. Currently Ashley is able to manage 8 cases.

CI Final — Ruth G. Mashak 10 hour day schedule a full caseload for a new graduate is 15 cases. Currently Ashley is able to manage 15
cases on her own with array of level of difficulty from an elective surgery to ICU cases.

Recommendations
CI Mid-Term — Ruth G. Mashak continue with current caseload with a combination of simple and complex cases and less supervision required.

CI Final — Ruth G. Mashak Ashley continue to do well and you will succeed in your future as a Physical therapist.
