R. Macayan, Jr., of legal age, Filipino and a resident of Tulo
1, Pinagpala, Batangas City, after having been duly sworn to
in accordance with law, hereby depose and say, that:

[1] I am the mother of JHON ALEXIS G. MACAYAN

who is a student of Lyceum of the Philippines University
(LPU) Batangas and an applicant for Senior High School
Voucher Program;

[2] I have no existing employment with any employer

since I am a plain housewife;

[3] I hereby attest that the source of my income for

sustaining the necessities of the family is solely the income
of, in the amount of P100,000.00 monthly, my OFW
husband;

[4] I am executing this affidavit to attest to the

truthfulness of all the foregoing facts and for any legal
purpose/s it may serve.
