To whom it may concern:

As a requirement for the renewal of LTO of JHIME DRUGSTORE- POTOTAN, I, Wianne Mae P.
Balatongle, fully understand the need to attend the FDA appropriate licensing seminar.
However, it was only last September 29, 2018 that I was able to take my oath as a licensed
pharmacist. This is the reason why I wasn’t able to attend the said seminar.

In connection with this, I would like to ask consideration from your good office to allow me
to proceed with the process. With full accountability, I promise to attend the said seminar in
the soonest possible time.
