B. Parent’s Contact: (Tell the parent’s name/phone number or how you

contacted him/her if no phone number or no answers)
I have already given my phone number to the students. Some students called
me when they were absent. Sometimes some parents call me to ask permission for
their child and some asked about their child study progress or absent…etc. So far, I
have already contacted a few parents about their child’s absence. I have also talked
with students who have most absences directly.
III. Students with special need (Talk about students who are so
weak/poor with their studying in class or their problems at home or school
which may affect our study at COERR, and the needs or assistance the
Center should provide.)
It seems like some students don’t care much about their study especially
workbook, and their absences. However, I try to persuade them to do it even though
it’s so late to submit or to come to class regularly. Moreover, I try to tell them the
advantages of coming to class regularly.
IV. Overall Comments & Follow Up (Talk about the results of the previous
actions)

After I warned them, a few of them come to class regularly and do their workbook.
Some are still the same. They don’t care much about their study.
V. Maintenance, General rules, and operation [Talk about repair or
replacement of equipment, implementation of the rules and policies, violations of
rules, discipline problems, safety issues, teaching ethics concerns of our teachers
(tardiness, sloppiness, commitments etc.), and general operation]
Everything is good, but there should have some maps at COERR2.
