FRESHMAN ADMISSION AND ENROLLMENT PROCEDURE


ACADEMIC YEAR 2019-2020

Step 1. SUBMISSION OF ALL REQUIRED CREDENTIALS FOR ADMISSION

Submit all the REQUIRED CREDENTIALS FOR ADMISSION to the Admission Officer, Office of Admission Services
(OAS/OUR), Ground Floor, W-107 West Wing, Sta. Mesa, Manila, on the date of your scheduled processing of credentials,
interview and enrollment as reflected in your SAR Form 1.

 Only applicants with complete credentials required for admission will be accommodated and shall go through the
admission process.

 Freshman applicants are advised to come for interview in appropriate attire and proper grooming (with collar and
sleeves; no rubber slippers, ‘tokong’/shorts; and no earrings for male).

 Non-appearance on your scheduled date of interview would mean automatic forfeiture of your priority
slot in the course of your choice, or even forfeiture of the slot in the University depending on the turnout of
PUPCET qualifiers. Admission of qualified applicants is on first come, first served basis. However, late applicants
may be re-scheduled or put on the waiting list subject to availability of slots.

 For applicants whose High School/ Senior High School English final grade is below 80%, PEPT and ALS A &
E Test qualifier, proceed to the Department of English, College of Arts and Letters, 2nd Floor, North Wing, PUP
Sta. Mesa Campus to take the English Placement Test (EPT). A fee of P150.00 for the English Placement Test shall
be paid to the Cashier.

Step 2. COLLEGE INTERVIEW AND REGISTRATION

Proceed to the College offering the program of your first choice for interview by the Dean or Chairperson. In case you are
not qualified or there is no more available slot in your first choice, you can go to the college of your second or third choice
until you get the approval for acceptance in your desired program.

Admission in PUP is on a FIRST-COME-FIRST –SERVED basis subject to the availability of slots.

Proceed to the PUP Medical Clinic, Ground Floor, East Wing, and present your Student Admission Record Form 1 (SAR F1)
to secure a medical clearance.

Step 4. ISSUANCE OF REGISTRATION CERTIFICATE


Submit your accomplished SAR Form 1 on the schedule date to PUP Office of Admission Services/Office of the University
Registrar (OAS/OUR), Rm. 107 Ground Floor, West Wing to get the printed copy of your Registration Certificate to be
announced during the enrollment period. Proceed to your assigned Admission Staff base on the first letter of your
surname.
NOTE: Present your Registration Certificate to your teachers during the first day of classes.

First Semester Classes start on June 17, 2019.

WHAT ARE THE ADMISSION REQUIREMENTS TO BE SUBMITTED?

A. Required Credentials:

When applicant passed the PUPCET, he/she must submit an original copy and photocopies of the following documents to
the Office of Admission Services:

For Graduating Senior High School Student


 Original or certified true copy of grade 10 & 11 high school card.
 Original Senior High School Card (F138) with school dry seal.
 Notarized certification of non-enrollment for high school/senior high school graduates who have not enrolled in
any technical/diploma/degree program immediately after graduation.
 Certificate of Completion for high school/ senior high school.
 Original PSA Birth Certificate.
 Certification of Good Moral issued by the HS/SHS Principal/or Guidance Counselor with school dry seal.
 Duly authenticated copy of the PEPT/ALS A&E Test Certificate of Rating, certified by the DepEd-NETRC -BALS
 School ID (for currently enrolled students) or any valid ID with picture (for previous SHS graduates)
 Current Chest X-Ray (PA) result with film (6 months validity from schedule date of medical clearance)
 Medical Clearance issued by the PUP Medical Clinic.
 Two (2) pcs. 2 in. x 2 in. colored picture with white background with applicant's name on the photo.
 One (1) pc. long brown envelope.
 Original Copy of Grade 10 High School Card or Certified True Copy with school dry seal.

In addition to the above admission requirements:
 Certification from high school/senior high school registrar with school dry seal and noted by the Principal that no
copy of F137A has been sent to other college or university.
 Notarized certification of non-enrollment for high school/senior high school graduates who have not enrolled in
any technical/diploma/degree program immediately after graduation with "waiver" that if there is concealment of
previous enrollment, PUP enrollment becomes null and void.
 For PEPT qualifier, a duly authenticated or verified copy of the Certificate of Rating and a Certification issued by
the Department of Education (CMO No.15, s. 2003)
 Duly authenticated copy of the PEPT/ALS A&E Test Certificate of Rating, certified by the DepEd-NETRC–BAL for
secondary level.

B. Other Requirements:

1. One (1) piece long, brown envelope.


2. For applicants whose Senior High School English final grade is below 80%, PEPT and ALS A & E Test qualifier,
proceed to the Department of English, College of Arts and Letters, 2nd Floor, North Wing, PUP Sta. Mesa Campus
to take the English Placement Test (EPT). A fee of P150.00 for the English Placement Test shall be paid to the PUP
Cashier.
3. For entrance scholars, Entrance Scholarship Committee Route and Approval Sheet duly signed by the members of
the concerned committee and one (1) photocopy of the credentials submitted for scholarship grant (Proceed to
Scholarship and Financial Assistance Services, Ground Floor, West Wing, PUP Sta. Mesa Campus for further
inquiries)
