Suffering from an existing disqualification.
Double or multiple registrant, or someone who uses the same name
repeatedly in the same or other precincts.
Already voted as shown by the indelible ink on the finger.
Received or expects to receive, paid, offered or promised to pay, contributed,
offered or promised to contribute money or anything of value as
consideration for his vote or for the vote of another.
Made or received a promise to influence the giving or withholding of any
such vote.
Made a bet or is interested directly or indirectly in a bet that depends upon
the results of the election.

denying the right to vote of Mr./Mrs. ___________________________________, on the

tagabantay ng partido/kandidato sa presintong nakasaad sa itaas, ay hinahamon ang BEI sa

sa sumusonod na kadahilanan)

Was not allowed to vote by the BEI despite presenting valid proof of
registration, identity or qualification.
Was not allowed to vote by the BEI despite identifying the voter through his
specimen signature in the EDCVL.
Was not allowed to vote by the BEI despite being identified by any member of
the BEI under oath.
Was not allowed to vote even after the challenged voter took an oath before
the BEI that he or she has not committed any of the acts alleged in the
challenge.
The voter was not allowed to review the scanned ballot in the LCD screen of
the VCM.
The voter was not allowed to re-feed the ballot 4 times in 4 different
orientations after his/her ballot was rejected during the scanning by VCM.
The voter was not given another ballot after his/her ballot was rejected by the
VCM for being an INVALID ballot.

of the precinct herein-above indicated is challenging the procedure/conduct of the
election, on the following grounds:

tagabantay ng partido/kandidato sa presintong nakasaad sa itaas, ay hinahamon ang proseso/
pagsagawa ng eleksyon, dahil sa sumusonod na kadahilanan)

The VCM box and the ballot box are NOT inside the polling place.
The BEI did NOT how to the watchers and the public present that the VCM
box is sealed; Ballot box is empty and thereafter, lock with four (4) seals; and
Package of official ballots is duly sealed, and thereafter, break the seal.
The BALLOTS CAST COUNTER was not ZERO “0” at the start of the voting.
There are Poll watchers who are not qualified
Persons prohibited to stay inside the polling place was allowed inside
without valid reason.
A voter was not allowed to protest the result of his/her voter’s receipt.
BEI did not allow voter to personally feed his/her ballot.
BEI did not allow voter to personally press the cast button or return button.
BEI did not enter voter’s/watcher’s complaint in the minutes
Voters were not allowed to vote even if present within thirty (30) meters from
the polling place by 5:00 p.m. of the Election Day.
A voter was allowed to bring the ballot, ballot secrecy folder or marking pen
or voter’s receipt outside of the polling place.
A voter was allowed to fill out his or her ballot accompanied by another, even
if not an illiterate or person with disability;
A voter was allowed to use capturing device, including, but not limited to,
digital cameras or cellular phones;
A voter allowed to use another ballot even if not “INVALID”
The BEI announced any of the following matters during the voting period:
whether a certain registered voter has already voted or not;
number of voters who have already voted or failed to vote thus far;
any other fact tending to show or showing the state of the polls; or
as to how many persons voted.
The poll watcher was not given a copy of ELECTION RETURN
A voter or any person disrupted or attempted to disrupt the normal
operations of the VCM.
