to in accordance with law hereby depose and state that:

1. I am a holder of a  non­professional/professional  driver’s license  with

2. Sometime on _________________, when I was at ____________________,
I checked my ______________________ for my driver’s license;

3. Unfortunately, then and there I discovered that the same is missing;

4. I searched my __________________________, but still the same, I cannot
locate the said driver’s license;

5. I asked my ______________________ but to no avail;

6. Despite   diligent   efforts   exerted   to   locate,   find   or   recover   the   above­

mentioned driver’s license, the same could no longer be found and may
now be considered as lost and beyond recovery;
