Person I, JUANA A. DELA CRUZ, of legal age, Filipino, and with residential address at 123
Oath Dapitan St., Sampaloc, City of Manila, after having been duly sworn in accordance
with law, hereby depose and state that:
Statement
Signature 1. I am an employee of ABC Corporation and was issued Employee
Jurat Identification (ID) Card No. 12345 (the “employee ID”) upon my hiring
on 30 September 2012;

2. Upon reaching the office last 22 November 2018, I realized that my


Employee ID is not in my possession;

3. I searched for my Employee ID in the office and in my residence, but


efforts to locate the same proved futile.

4. I requested the Human Resource Department to issue a replacement


Employee ID and I was informed that I need to execute this Affidavit.

5. I am executing this Affidavit of Loss to attest to the circumstances of


the loss as part of the requirements in securing a replacement ID from
the Human Resource Department.

Witnesseth that-

Whereas, the Seller is the true and absolute owner of a parcel of land with
improvements located in España Boulevard, Sampaloc, City of Manila with an
area of four hundred square meters (400 sq.m) more or less and covered by
Transfer Certificate of Title No. 12345 issued by the Registry of Deeds for the City
of Manila (hereinafter the “Subject Property”).

Whereas, the Buyer desires to purchase the Subject Property and the Seller is
willing to sell the same to the Buyer, subject to the terms and conditions of this
Deed of Absolute Sale.

NOW THEREFORE, for and in consideration 5 of the foregoing premises, and the
amount of Twenty Million Pesos (P 20,000,000.00), receipt of which in full is
hereby acknowledged by the Seller, the Seller hereby SELLS, TRANSFERS and
CONVEYS6 the Subject Property, absolutely and irrevocably, in favour of the
Buyer, her heirs and assigns, together with all the rights and interests thereto,
free from all the liens and encumbrances.
