AFFIDAVIT OF SERVICE

I, PERICLEO SOLIS, of legal age, Filipino, single and with office address at Suite B 2nd
Floor Overland Park Bldg., No. 245 Banawe St. cor. Quezon Ave., Quezon City, after having been
duly sworn to in accordance with law, depose and state:

1. That I am the associate of ATTY. PEARLITO B. CAMPANILLA, the counsel of record in the
present petition;

2. That I served on _________________ a copy of the pleading in the foregoing case entitled
“COMPLIANCE” together with the contents thereof by REGISTERED MAIL
in accordance with Rule 13 of the Rules of Civil Procedure and Sec. 6 of A.M. No. 11-9-4-
SC by depositing a copy of the aforesaid Petition in the post office in a sealed envelope,
plainly addressed to the following:

undelivered. The aforesaid pleading was sent to the Hon. Court and the above-named party
by registered mail in view of the worsening traffic situation in Metro Manila and lack of
manpower to effect personal delivery.

3. That on _________________, I served a copy of the said pleading with the Honorable
National Labor Relations Commission (NLRC) located at PPSTA Bldg., Banawe St. near
Quezon Ave., Quezon City by PERSONAL SERVICE by delivering personally a copy of the
petition at the docket section of the NLRC together with the contents thereof as duly
evidenced by the received stamp on the original copy of the pleading.
