On 11/19/18 I went to Childfind in Puyallup and was allowed to follow a family as their child was given a

preliminary assessment. I observed a shy 4.8-year-old girl. She is the youngest of four siblings and
Ukrainian is her primary language. She attends a private preschool and Mom’s primary concern is her
communication and speech skills.

Once the child was all checked in, I followed the mom and daughter to the different rooms. The first
room was for hearing. The girl cried, and we went on to the speech language pathologist (SLP) and vision
check. The SLP asked the girl to say what was pictured and the girl said nothing. The SLP tried 5 more
times, scoring the girl 0/5. The SLP then asked the girl to point to the apple, and the girl did. The SLP
then asked her to point to 9 other objects and she got them all correct scoring 10/10. The SLP asked the
mom if the girl speaks more than 50 words in English and/or Ukrainian, the mom said yes but more
words in her primary language and was difficult to understand in which ever one she was speaking in.
The girl did not speak the entire time we were in there but showed great receptive language skills. For
the vision, the SLP used a spot screener and came back normal.

Next, we went back to hearing and the girl sat down in the chair after many prompts from the mom. The
girl allowed the audiologist to put the earphone on her and showed her how to raise her hand when she
heard a beep. The girl passed the hearing test.

Next was gross and fine motor. This room is more like a playroom, with stairs and toys along with tables
for working at. At first the girl did not participate. Her mother prompted her to walk up the stairs, and
she did. Next, she was to walk on line, toe to heel. She did that with ease. Finally, she was instructed to
jump into a square, she stood by her mom and did not participate. Then she was instructed to throw a
beanbag at a target and she again stood by her mom and did not participate. Next, the girl was taken
over to the table to work on fine motor. She did not participate in any activities.

We left the room and waited for the final evaluation, cognitive. While we waited the mother of the girl
asked if her daughter could watch another child do the tests as a peer model. Once the girls saw the
other girl do all the tests, she participated in all the gross and fine motor tests, passing all above
average.

Next, we went to the cognitive evaluation. The girl still has not said a word and was asked to name a
picture of an apple. She did not speak, and the proctor got another visual assessment tool. She was
asked to point to certain items, and then to her own body parts. She passed all tests.

Finally, it was time for the exit meeting. The girl qualified for communication and will be returning for a
full evaluation for communication.
