 As a probationary employee, with the position of [position] you shall report directly
to the [position] or his/her designate and will work in the [branch/location] or in any
assignment depending on the operational requirements of the company.

 The scope of your work, duties and responsibilities are in the accompanying job
description/responsibilities. Nevertheless, the company reserves the right to amend,
revise or modify the same as amended by the nature of the company’s operations. In
addition, you attest by your signature below that the standards for you to attain
regular employment have been fully and completely explained to you in a language
known and understood by you, with you understanding the full and complete
practical and legal implication of the same.

 Your employment shall be on a probationary status. However, you understand that


your employment may be terminated In accordance with the provisions of this
Employment Agreement, the Company Policy/Rules and Regulation and such
grounds the Labor Code of the Philippines and relevant laws may provide. The
probationary period shall commence upon full and complete submission of all pre-
employment requirements. However, the company may allow you to render services
under a project basis, with the project period being on a day-to-day and your failure
to submit the said pre-employment requirements shall be sufficient ground to
terminate this contract immediately.

 In the event that your services are terminated during the effectivity of this contract,
you will be entitled to collect only salaries up to the end of the working hours of the
last day of actual service and prorated 13'" month pay. Should you decide to resign,
you shall give us a written notice of at least thirty days (30) before the intended date
of resignation. It is understood that days spent with the company in completing
clearance procedures shall not be computed as part of salaries earned.

 You are to keep company’s confidential information such as records, documents and
processes pertaining to its business or affairs with the understanding that no
unauthorized disclosures or reproduction of the same will be made by you at anytime
to anybody nor otherwise expressly authorized in writing by the management, during
or after your employment with the company. Any failure to comply with the
foregoing shall be ground for dismissal without prejudice to the filing of the
appropriate criminal and/or civil cases against you.
 It is likewise agreed that as a [position], you are given the full trust and confidence
of the company in handling all inventory and goods for sale; all funds such as sales
proceeds, petty cash and the like; all company property and equipment. Thus, should
you commit any theft or fraud against the company, the same shall be tantamount to
qualified theft, without prejudice to the filing of other appropriate criminal violations
against you under the Revised Penal Code and/or All Special Penal Laws.

 It is likewise understood that any variance in sales and/or inventory and/or damage
to company equipment and/or property is sufficient grounds for termination and you
shall IMMEDIATELY be placed under preventive suspension within the maximum
period allowed by law while the case is being investigated. Further, the disciplinary
process shall not be a bar to the filing of any and all criminal and administrative
proceedings against you.

 As a probationary employee you are entitled to the following compensation and and
government mandated benefits, subject to deduction for taxes and government
mandated employee contributions, to wit:
A. Premium Pay
B. Holiday Pay
C. Maternity or Paternity Leave
D. 13TH Month Pay

 It is likewise agreed that all intellectual property creations that are established during
your employment, in consideration of your salary, shall be owned exclusively by the
Company. Company secrets and business trades are to be kept confidential. You are
not allowed to be employed in a competitor or entity engaged in allied and/or
connected business within 1 year from cessation of your probationary employment.
Violation of this shall entitle the company to liquidated damages in the amount of
P100,000.00 and your final pay and cash bond shall be withheld to answer for this
liquidated damages.

 As a person who holds cash and other valuable properties, the law allows that you
shall be covered by a cash bond in the amount of P10,000.00. This cash bond shall
be deducted from your bi-monthly pay in the amount of P50.00 per pay cycle until
such time as the amount is reached. Any and all amounts deducted shall be kept in a
separate account under your name. The same shall be immediately released to you
after clearance process and a determination that you have no pending liability with
the company.

 You are being hired on the basis of your representation that you have met the
physical and mental health requirements and that you have not been charged with
nor convicted of any criminal offense, or is presently involved in any such criminal
and/or civil case. In addition, your employment is based on your representation that
- if applicable - you have been duly qualified to practice your profession and during
the course of your employment shall continue to be so qualified and perform all acts
necessary to maintain such qualification including but not limited to continuing
education as required.
 You agree to maintain the foregoing qualifications in the future, and it is understood
that should you fail to maintain the said requirements in the course of your
employment with the company, your services shall accordingly be terminated.

This document constitutes the terms of your employment. It is understood and agreed upon that
the services to be rendered by you under this agreement shall be performed at all times to the
satisfaction of the company.

STANDARD FOR REGULARIZATION

For you to be regularised you must be able to exhibit satisfactory compliance with the job
description that follows.

The Company places a premium on professionalism and any minor infraction of this policy
shall be a ground for non-regularization. In addition, the employee is expected to be punctual
at all times and no excuses shall be allowed for unauthorized lates and absences, which are also
grounds for non-regularization.

Broad Function

task as maybe assigned from time to time.
Principal Responsibilities

1. Welcomes customers by greeting them, offering them assistance.

2. Guide customers by providing information on products.

3. Helps customer make selection by building customer confidence, offering suggestions
and opinions.

4. Directs customers by showing the shelves or bins; suggesting items with marketing
promotions.

5. Process payments by totalling purchases; processing cash or credit cards and e-wallet
payments.
(Paraan ng pagbabayad kwentahin ang mga pinamili ito man ay sa pamamagitan ng cash

6. Records and monitors all sales transaction in proper mediums (CSI, POS, or ESR).

7. Ensures to meet daily sales targets in conformity to monthly quota.

8. Receives safe keep and transmit cash and other forms of payment.

9. Full compliances to basic store standards such as but limited to cash controls,
merchandise replenishment, display and presentation, cleanliness and orderliness of stores, etc.

10. Performs daily, weekly and monthly inventory.

11. Properly checks and receives deliveries.

12. Maintain legal and actual possessions of all items to be sold with the direct responsibility
to safe keep the same and maintain its physical and legal integrity.

13. Perform reportorial functions of team leader in her absences, e.g. compute and fill up sales
monitoring forms, send sale to supervisor.

subaybayan ang iyong linalagay).
14. Assist Team Leader in the execution of business process (POS, ORW, etc.)

15. Performs other duties as may be assigned from time to time.
