PREFACE

1.1 STATEMENT OF INTENT


1.1.1 EXPRESS AIR LIMITED has develop this Emergency Medical Services [EMS] Manual to provide
technical guidance for its personnel engaged in the planning, flying, inspecting of aircraft and stretcher
installation, and maintenance. All EXPRESS Air Limited personnel shall be mindful of all Bahamas Aviation
Regulations (as revised) for EMS operations in addition to those required for commercial operations.

1.2 FOREWORD
1.2.1 This manual provides additional operational and maintenance requirement to be followed by
EXPRESS Air Limited whiles conducting Emergency Medical Services (Special commercial operations).

1.2.2 EXPRESS AIR LIMITED Flight Crews shall ensure the aircraft [C6-FLI] is operated using the correct load
manifest and weight & balance. The installation and removal of the stretcher shall only be performed
individuals appropriately trained and authorized by the company. Failure of any EXPRESS Air Limited operation
or maintenance personnel to comply with the regulations, requirements and procedures outline for EMS
operations shall result in removal of authorizations and approvals until retraining has occurred. Repeat
offender shall be permanently removed from EMS operations.

1.2.3 The Director of Maintenance shall ensure EXPRESS Air Limited personnel are properly training prior
to being authorized to install and remove the stretcher system. The system will require the removal of seats
prior to the installation of the stretcher using the seat track.

1.2.4 The Chief Pilot will ensure all pilots receiving additional briefing/training prior to being assigned EMS
duties. All pilots who are called out, and/or called out and fly EMS flights shall include from the time the call is
received until arriving back at home as duty time. All time flown shall be calculated using the same formula
used for calculate their flight time in commercial transport.

1.2.5 The Chief Inspector shall incorporate the Continuing Airworthiness Inspection into the EXPRESS Air
Limited Maintenance Program for the Beechcraft 99 aircraft (C6-FLI). The Chief Inspector shall ensure the
Director of Maintenance complete the required training prior to the issuance of authorization to install and
remove stretcher system.

1.2.6 As various company manuals are revised to reflect changing policies and procedures, minor
inconsistencies may become evident. It is the manual holder’s responsibility to contract supervisory personnel
for clarification. Where conflicts occur, the regulations [BASRs/FARs] shall take precedence over the company
operations manuals and procedures as appropriate.

1.2.7 Suggestions for changes to this manual that enhance safety, passenger convenience, or efficiency of
operation are welcome and shall be considered. Any requirements for changes to this manual by the Authority
[FSI/FAA] (when required by law/applicable) will be accomplished within the prescribed time frame.

1.2.8 Company Officers and supervisors shall supplement this manual when required with detailed
instructions to enhance the effectiveness of daily operations.

1.2.9 EXPRESS AIR LIMITED Director of Operations or designee shall be responsible for the distribution of
this manual and for providing the revision service. A master list of all manuals is maintained by the
Accountable Manager or his designee. It is the responsibility of the manual holder to post the revisions and log
the changes on the Record of revisions page. With every revision issued to each manual holder, a return
receipt will be enclosed. The manual holder is responsible for completing and returning the receipt to EXPRESS
AIR LIMITED.

1.3 GLOSSARY
Aircraft Technical Log. A document attached to an aircraft for recording defects and malfunctions discovered during
operation and for recording details of all maintenance carried out whilst the aircraft is operating between scheduled
visits to the base maintenance facility. It also contains operating information relevant to flight safety and maintenance
data that the operating crew need to know.

Acceptable to the Authority means a portions of documents, formal procedures, facilities, equipment, or personnel that must be
found satisfactory by a technical review of the Authority prior to use in aviation.

Approval means a formal document issued by the Authority based on a prior technical evaluation that authorises the use of
documents, portions of documents, or formal procedures in aviation.

Approved by the Authority means as used in these Schedules, a document, person, facility, policy or procedure for which there
must be a formal document issued prior to their use in aviation. The separate use of the word “approved” in these Regulations
shall be assumed to be “approved by the Authority”

Authorised by the Authority” means t his phrase identifies a requirement that involves a technical evaluation and the issuance of
a formal document of authorisation.

Authorisation means a formal document issued by a civil aviation authority based on a technical evaluation that officially
conveys to the holder certain privileges in aviation under the civil aviation law, Regulations and Schedules.

Aviation personnel means aviation personnel shall include persons whose duties are directly related to ensuring aviation safety
and security, aircraft operation, air transport, and air navigation. They shall possess licences and certificates which are issued or
validated by the Authority.

Calendar day means the period of elapsed time, using Coordinated Universal Time or local time, that begins at midnight and
ends 24 hours later in the next midnight.

Calendar month means a period of a month beginning and ending with the dates that are conventionally accepted as marking
the beginning and end of a numbered month (as January 1 through January 31 in the Gregorian calendar).

Calendar year means a period of a year beginning and ending with the dates that are conventionally accepted as marking the
beginning and end of a numbered year (as January 1 through December 31 in the Gregorian calendar).

Commercial air transport operation means an aircraft operation involving the public transport of passengers, cargo or mail for
remuneration or hire.

Flight duty period means a period which commences when a crew member is required to report for duty that includes a flight or
a series of flights and which finishes when the aircraft finally comes to rest and the engines are shut down at the end of the last
flight and there is no intention for further aircraft movement by the same crew member—
(i) A flight duty period includes the duties performed by the crew member on behalf of the operator that occur before a
flight segment or between flight segments without a required intervening rest period;
(ii) Examples of tasks that are part of the flight duty period include deadhead transportation, training conducted in an
aircraft or flight simulator, and airport/standby reserve, if the above tasks occur before a flight segment or between flight
segments without an intervening required rest period.

1.5.1 EMS revisions will be sent to all manual holders. Upon receipt of a revision the manual holder shall insert
the revision page (s) in the manual and destroy all pages (s) that have been revised. A master list of all manual holders
is maintained by the Chief Inspector and/or the Director of Operations.

1.5.2 This record shall be retained in each approved copy of EXPRESS AIR LIMITED Emergency Medical Services
Manual.

1.6 REVISION PROCEDURES

1.6.1 Revisions to and distribution of the manual are the responsibility of the Accountable Manager or
his/her designee. Proposed revisions will be forwarded to the Authority [FSI/FAA], as appropriate, for
approval, acceptance and/or review. No person may act upon any proposed revisions to this manual prior to
approval, acceptance and/or review by the appropriate Authority.

1.6.2 Distribution of the revision will be via company mail or by common carrier. EXPRESS AIR LIMITED
shall distribute the revisions to the latest address that has been provided by the manual holder. It is the
responsibility of the manual holder to keep this information current at all times.

1.6.3 Apparent missing revisions are to be brought to the attention of the Accountable Manager or
his/her designee for correction.

1.6.4 Each revision will be accompanied be a “List of Effective Pages”, a confirmation of revision change
receipt and instructions as to the manner in which the revision is to be accomplished.

1.6.5 Each numbered paragraph that contains a significant change will be denoted by a vertical black bar
along the left-hand margin, indicating an addition or deletion.

1.6.6 Special attention should be directed to these paragraphs during manual updating.

1.6.7 Once the update is completed the revision change receipt shall be completed and returned to
EXPRESS AIR LIMITED.

2.1 Regulatory Requirements

2.1.1 Section 18.285 reads:


(a) A person may handle or transport dangerous goods within The Bahamas by an air ambulance
dedicated to and configured for the transport of patients, of persons who are accompanying or
who have accompanied a patient or of medical personnel if—
(1) The transport of the dangerous goods is not forbidden by Technical Instructions;
(2) The dangerous goods are contained in a means of containment that—
(i) Has displayed on it the package markings and labels required by the ICAO
Technical Instructions;
(ii) For a cylinder, is in compliance with containment requirements; and
(iii) Is secured to prevent movement during transport.

This is a dangerous goods regulation that speaks to a dedicated to and configured for the transport of
patients; however, EXPRESS Air Limited aircraft [C6-FLI] is only configured for Emergency Medical
Services when a call is received.

EXPRESS Air Limited Aircraft [C6-FLI] normally remain in a 15 seat passenger configuration.

Beyond Section 18.285 there are no additional regulations which address Air Ambulance operations.

EXPRESS Air Limited has therefore derived its procedures using examples, guidance and standards
taken from other ICAO Contracting States.

2.2 Flight Crew – Airplane Training Program

2.2.1 The following elements should be considered in an operator's airplane flight training
program and airplane ground training program.
a) Airplane Flight Training Program.
i. There are no airplane flight training maneuvers which are unique to air ambulance
services.

b) Airplane Ground Training Program. The following areas should be included in the operator's
ground training curriculum:
i. Aircraft systems variations.
ii. Passenger restraining methods in flight.
iii. Handling problem passengers.
iv. Cockpit crew functions and responsibilities.
v. Prevention and control of infections conditions (to include knowledge of infections and
communicable diseases, diseases recently identified as occupational health risks such
as hepatitis B, herpes simplex , herpes zoster , and AIDS).

2.3 Medical Personnel & Cockpit Crew Coordination Training

2.3.1 In air ambulance service operations, the medical personnel and the cockpit crew are
involved in two distinct operations. While cockpit crewmembers are generally not required to be
trained in any medical subject areas, and medical personnel are not required to be trained in any
flight operational procedures, training is permitted in some specific areas that the operator deems
necessary.

2.3.2 Medical Personnel Not Considered Crewmembers.

Medical personnel are not considered crewmembers by EXPRESS Air Limited. The medical personnel are
not required to complete initial and recurrent crewmember training programs.

The medical personnel shall not perform any duty in an aircraft that relates to the operation of that aircraft,
such as assisting the flight crew in seeing and avoiding other aircraft or emergency shutdown of aircraft
systems in a crash.

If the medical personnel is considered to be passengers by EXPRESS Air Limited; when medical
personnel, a patient or passenger are on board the aircraft, the flight shall be conducted in
compliance with and under all Bahamas Aviation Regulations requirements, company operations
specifications, authorization and approvals enforce.

2.3.3 Operators must have training in coordination procedures to be used between cockpit
crewmembers and medical personnel in specified situations.

These specific situations must include, but are not limited to, the following:
a) Physiological aspects of flight.
b) Patient loading and unloading.
c) Safety in and around the aircraft.
d) Passenger briefing (when appropriate).
e) Appropriate in-flight emergency procedures.
f) Emergency landing procedures.
g) Emergency evacuation procedures

2.4 Dangerous Goods Training

2.4.1 Flight Air Limited Flight Crew Training Program complies with the Bahamas Regulations
requirements and as shall all pilot receive initial dangerous goods training (Section 14.070). The
aspects of this training that addresses labeling, markings and packing/containers shall be emphasize
due to the likelihood of these items being transported during EMS operations for patient’s care.

2.5 Flight Crew Responsibilities

2.5.1 The assigned flight crew responsibilities remain the same as a normal commercial
operation. Captain / PIC is responsible for the safe operations of the aircraft and shall direction all
none medical activities.

Prior to passengers boarding the aircraft a flight crew member shall inspection the stretcher system
to confirm its security.

The Captain shall obtain a briefing from the medical personnel on the medical condition of the
patient in as much as is required to determine the patient’s condition and would it could be adversely
affected by what would otherwise be considered normal operations:
1. Rate of Climb
2. Rate of Descent
3. Cruise flight level
4. Rate of turn / bank
5. Possible weather

Once the normal checks are completed and an engine is started one pilot shall remain in his duty
position to monitor the operations of the aircraft.

The other pilot shall be responsible for the briefing of all passengers and the securing of the cabin
door. This pilot shall also ensure the proper stowage of articles inside the cabin and the proper
security of the patient fitted to the stretcher.

Once airborne company radio communication shall be carried out as per operations manual along
with require for an ambulance to be present upon arrival.

Additionally, the flight crew shall periodically communicate with the medical personnel whiles in the
air.

After landing again company radio communications shall be carried out as per operations manual.
The flight crew shall be allowed to confirm the presence of an ambulance after receiving
confirmation of gate assignment.

One Pilot shall again remain at his duty station while the other pilot operates the passenger door and
assist the medical personnel.

Once the mission is completed the flight crew shall reposition the aircraft to a position where
cleaning and necessary precautions can be taken / perform before the aircraft is returned to regular
passenger handling and commercial air transport operations.

3.1 SERVICING

The Director of Operations shall coordinate with maintenance to ensure the aircraft is deep cleaned
and seat covers shall be changed if needed.

If the Director of Operations have knowledge for the medical personnel or have a reasonable
concern that an airborne virus/bacteria or a communicable illness may have been associated with
the patient transported he shall advise maintenance.

Maintenance shall take every precaution by suiting up with bio-hazard suit prior to cleaning. Should it
be necessary the aircraft manufacturer shall be consulted for cleaning of air condition system and
filters.

EXPRESS Air Limited shall not return C6-FLI back to normal operations until it is satisfied all
required steps has been taken to disinfect and clean the aircraft has occurred.

Maintenance shall make an entry stating the aircraft has been cleaned in the aircraft technical and
journey log.

3.2 MAINTENANCE

3.2.1 The Chief Inspector shall revise the maintenance schedule to include the continuing
airworthiness instruction inspections associated with the installation of the Stretcher system [STC].
The Chief Inspector shall also check FAA Airworthiness Directives to ensure continued compliance.

If an AD is found for the installation of this stretcher system it shall be incorporated into the
maintenance program.

3.2.2 The Director of Maintenance shall liaise with the Chief Inspector to ensure the continued
airworthiness instruction inspections has been incorporated into the maintenance program.

The Director of Maintenance shall ensure the aircraft has been cleaned and disinfected once EMS
mission/operations have been completed.

Should it become necessary to contact the manufacturer the Director of Maintenance shall liaise with
the Chief Inspector to ensure the maintenance recommended by the Manufacturer is accomplished.

The Director of Maintenance or his designee shall be responsible for ensuring once the call is
received the necessary seats are removed and the stretcher is properly installed.

Once the EMS flight(s) has been completed the Director of Maintenance or his designee shall
ensure the aircraft is cleaned and re-configured for normal commercial air transport operations.

3.3 MAINTENANCE TRAINING

The Director of Maintenance shall be responsible for ensuring all assigned maintenance personnel
has been training in the installation and removal of the stretcher system.

Additionally, maintenance personnel involved in the cleaning and disinfecting of the aircraft shall be
given addition instructions.

The Director of Maintenance shall review the techniques documented in the continued airworthiness
instruction document with the maintenance stuff.

Finally, the Director of Maintenance shall ensure all additional training is documented and supply to
the Chief Inspector.

4.1 FLIGHT DUTY PERIOD

4.1.1 EXPRESS Air Limited shall comply and ensure its flight crews continue to comply
with the requirements of Schedule 15 Subpart C - Schedule 15.035 and Appendix 1 to
Schedule 15.035 for multi-crew operations.

Prior to a EMS flight is assigned each pilot flight time duty record will be reviewed to ensure
the assigned pilot can complete the intended flight(s) while remaining in compliance with the
associated regulations.

Once a EMS flight has been complete the Director of Operations shall ensure the necessary
rest period is provide to the flight crew in accordance with Schedule 15 Subpart D.

All flight time, duty time and rest period shall be documented in the usual method as
prescribed by EXPRESS Air Limited Operations Manual.

5.1 EMS SUPPLEMENTAL TRAINING PROGRAM

5.1.1 EXPRESS Air Limited shall comply

BEECH 99 – C6-FLI

Minimum Time Required: 4.0 hours for each aircraft initial and 2.0 hours recurrent

Objective: To provide company, medical and security personnel training on safety in and around the
air ambulance aircraft before, during and after flight.
Purpose: This curriculum segment will give the company, medical and security personnel training in the
safe management of a medical evacuation.
Courseware: Aircraft Flight Manual, Aircraft, Stretcher, Medical Equipment.

Instructional Delivery Teaching methods for this segment are lecture, demonstrations, Operations Manual, Training
Methods: Manual, Drills.
Equipment/Training Aircraft, Stretcher.
Aids:
Subjects: See subjects listed under "C. Subject Areas".

A. Personnel to be trained:

1. Flight Operations Personnel.

2. Medical personnel who regularly or occasionally fly on the aircraft.

3. Emergency department personnel who have direct contact with flight program
and regularly or occasionally assist near the aircraft.

4. Security personnel who are regularly or occasionally assigned or provide security for
the aircraft or assistance to flight crews.

5. Replacement of any company personnel.

6. Maintenance personnel.

7. Any other person who will have occasional or regular direct involvement with the aircraft system
and will work in close proximity to the aircraft.

Medical personnel are considered to be passengers, not crewmembers. All flights conducted with medical
personnel on board shall be conducted under Schedule 12 and all applicable regulations.

B. Frequency of Safety Training:

1. All personnel listed above will receive training on areas as outlined at the beginning of program
evolvement while performing any duty requiring operation in close proximity to the aircraft or
requiring knowledge of aircraft operations. Training will be documented on the Ground Training
Attendance Record and the Record of Training.

2. Any personnel assigned after start of program will be briefed before performing any duty requiring
operation in close proximity to the aircraft or requiring knowledge of aircraft operations.

3. When significant safety policies/procedures changes are introduced and/or annually thereafter.

C. Subject Areas

1. Paragraph 3 below is an outline of subject areas in this section. The outline


shows who is to receive training.
2. Initial and recurrent training will be given in each subject area, according to the
following code:

P - Pilots

M – Medical Doctors, Nurses or Paramedics to be carried onboard

S - Security personnel or any other person who will work in close proximity to the aircraft in
loading, unloading, such as emergency department doctors, nurses or paramedics.

D - Maintenance personnel
