EMPLOYMENT CONTRACT

This EMPLOYMENT CONTRACT is executed on _____ day of _________________


2019 and entered into by and between:
RELUX WELLNESS LOUNGE (hereinafter referred to as “Employer”) is a domestic
corporation established and existing under the laws of the Republic of the Philippines and with
principal office address at Maharlika Highway, Villasis, Santiago City.

_____________________________, (hereinafter referred to as “Employee”), single, Filipino
citizen and with residence address at
___________________________________________________.
The “Employer” decides to employ the “Employee” and the “Employee” has agreed to
accept and enter such employment upon the Terms and Conditions set out in this agreement.

Terms and Conditions:


The parties of this “Contract” voluntarily bind themselves to the following terms and
conditions:

I. DATE OF COMMENCEMENT AND TERM

1. The employee will commence employment with the employer on the _____ day of
_______________________ 2019.

2. The employee shall be engaged with the employer on a probationary status for a
period of ______________________ months, within this period, the employer
shall observe the performance and attitude towards work of the employee as well
as his working relationship with his co-employees and employer.

3. Before his permanency in the Company, the employer shall review and evaluate
the over-all performance of the employee specifically in terms of punctuality,
relationship with his co-employees and employer, self-motivation, discipline,
adaptability, decisiveness, cooperation, honesty, timeliness submission of
outputs, efficiency and effectivity.

4. This Employment Contract shall be effective from the time both parties affixed their
signatures on the last page of this agreement.

5. The working days are from Monday to Saturday with working hours starting at
_____________________ in the morning and ending at
______________________ in the afternoon. This is equivalent to Eight (8) Hours
of effective working per day.
II. JOB TITLE AND DESCRIPTION

7. The duties and responsibilities of the employee will be the following:

Handles cash collection for remittance on a daily basis.
8. The employee will perform any and all duties as requested by the employer that
are reasonable and that are customarily performed by a person holding a similar
position in the industry or business of the employer.

9. The employee agrees to abide by the employer’s rules, regulations,


and practices including those concerning work schedules, vacation,
personal and sick leave, as they may from time to time be adopted or
modified.

III. EMPLOYEE COMPENSATION AND BENEFITS

10. Compensation paid to the employee for the services rendered to the Company as
required in this Agreement will include a wage at the rate of
_______________________ pesos (P__________) per month.

11. The compensation will be payable twice per month while this Agreement is in force.

12. The employee’s basic pay and benefits will be reviewed upon your permanency.
Any increase/adjustments to them shall be based on the employee’s performance
evaluation.

13. Overtime pay, Holiday pay and 13th month pay shall be based on the provisions of
the labor code of the Philippines and Department of Labor and Employment rules
and regulations.

14. The employee agrees to keep his compensation confidential and will not discuss
it with anyone within and outside the Company.

IV. CASH BOND

V. WORKING PLACE

15. The employee’s primary place of work will be at Relux Wellness Lounge, Villasis,
Santiago City.
VI. WORKING HOURS

16. The employee’s normal hours of work is six (6) days per week, eight (8) hours per
day from 10:30 AM to 7:30PM.

17. The employee agrees to a lunch break of one hour. The employee’s meal break
shall not be counted as working hour.

18. The employee is entitles to not less than 1 rest day in every period of seven (7)
days.

19. Any work on Sundays will be by agreement between the parties from time to time.
If the employee works on a Sunday he/she shall be paid the standard amount
provided by the Labor Code.

20. The employee will be entitled to all official public holidays in full pay. If the
employee does not work on a public holiday, he/she shall receive normal payment
for that day. If by any chance the employer asks the employee to work on a public
holiday, he/she shall be entitled to a double pay.

21. The employee is entitles to paid “annual leave” according to the provisions of the
Employment Guidelines.

22. The employee shall be entitled to a paid Maternity Leave/Paternity Leave


according to the company rules and regulations.

VII. NON-COMPETITION

VIII. TERMINATION OF CONTRACT


23. The employee reserves the right of the employer to terminate the employee when
a just cause arises as permitted and in accordance with the Labor Code of the
Philippines.
24. Either party can terminate this agreement with _______ days written
notice. In the case where an employee is illiterate, notice may be given by
that employee verbally.

IX. OTHER GOVERNING LAWS

25. Matters not provided for in this contract shall be governed by other applicable laws
such as the Labor Code of the Philippines, Revised Penal Code and interpretations
thereto by governing bodies.

26. Employees are prohibited from realigning funds or property from


which they are originally intended, or from misappropriating the same,
or from consenting to such misappropriation without the consent of
the Board of Directors or of the President.
27. Employees are required to comply with the highest form of ethical and
moral standards.

Employees are expected to refrain from committing crimes and/or


felonies such as corruption of public officer, bribery, or from
conniving or consenting to the commission of the same.

X. GENERAL PROVISIONS

28. This agreement shall be construed and governed in accordance with the laws of
the Philippines.

29. Any changes to this agreement will only be valid if they are in writing and have
been agreed and signed by both parties.

30. The parties shall observe the terms and conditions of this Agreement. Should there
be any DIFFERENCES in the interpretation of the same, the parties shall settle
and resolve this, to avoid lawsuit in a Court of Justice.

The Employer and Employee hereby declare that they understand thoroughly the above
provisions and agree to sign and to abide by such provisions. They shall retain a copy of this
contract for future reference.
