Understanding dental
histology

-When making anterior

restorations, enamel and/
or dentin have to be
restored
- Reflection and refraction
in enamel and dentin are
different.

Opacity and Translucency
-Opacity is the quality of a material that does not allow light to pass through it.
-Transparency is the contrary concept.
-It is the property of transmitting light without appreciable scattering so that
bodies lying beyond are seen clearly
Opalescence

• Depending on the type of light and its direction, the appearance of the teeth can change. This
effect is mainly because of the opalescence of enamel.
• Opalescence is a type of dichroism seen in highly dispersed systems with little opacity.
• The material appears yellowish-red under transmitted light conditions and blue under incident or
scattered light perpendicular to the transmitted light.
SHADE TAKING

- You could use the VITA shade guide . But if you want accurate shade match, you
should use the shade guide of the composite rather than the VITA.
TRANSLUCENCY OF THE DIFFERENT TYPES OF RESIN
COMPOSITE
SOME COMPOSITE
SYSTEMS

The different brands of

composite have
different names of
composite. But there
are usually devided
into four type : dentin,
enamel,body,
translucency.
WHY DO WE DO LAYERING

• Control thickness of each layer.

• Manage effects of polymerisation shrinkage.

• Maximize strength and aesthetics.

• Create natural tooth anatomies.

The layering techique. The enamel shade 0.5mm is recommended
You could use the LM Misura to control thickness thickness of enamel layer.
• Basic anterior
tooth anatomy
• The shape
improves the
shade !

• POLISHING

• Fine burs
• Enhance
polishing system
• Spiral Polishing
Wheels
• Groovy Diamond
Polishing Brushes
• Diamond polish
paste
CASES REPORT
All the cases aren’t perfect. But I have
found that I always learn more from
my mistakes than from my successes.
Case 1: -CC:A 30-year-old female patient wanted to restore the discolored old restoration.
Pulp test : +
X rays normal
Diagnostic : Secondary caries (Class IV Black Caries Classification )
-Tx:
+ Shade matching: A3
+ Isolation, direct mockup to restore incisal edge, take impression to create index
+ Restore palatal wall via index, restore mesial and distal sides with matrix retainers
(shade CE)
+ Continue with layering: OPA2, OA3, CE tokuyama

+Polish with Solflex

Intital Make platal and interproximal shell

Selective etching Final

Case 2:

-CC:-A 22 year old female patient had

tooth 21 was fractured after a trauma and
then stablized (needs to follow-up the
pulpal and periodontal diseases)
Pulp test : +
She have sensitive when using cold
drink.
Diagnostic: Fractured enamel and dentine
-Tx:
• Shade matching A2
• Isolation, Enamel Conditioning, Bonding
• Restore palatal, mesial and distal sides
with matrix retainers. (CE)
• Continue with layering :
OPA2,OA2,C(Tokuyama)
• Polishing with Solflex and Two-Step
Composite Polishing System and
Diamond Polish paste
• Rehydration assessment a week after
