score = 1-3 – no further action required, but follow and maintain safe working practices.
score = 4-5 – action should be taken to ensure that control measures are in place within 1 month
of hazard identification.
score = 6-8 – work should not be started until control measures are in place. If work is already in
progress, action should be taken as quickly as possible.
score = 9+ – work must not start until control measures are in place to reduce the risk. If this is
not possible then work may not begin. If work is already in progress it must be stopped
immediately.

NOTES ON SCORING RISK SEVERITY

Any hazardous operations involved in a procedure should first be identified. Each hazard
identified is scored on a scale of 1 to 3 under three categories:

Severity – the severity of the hazard


Probability – the probability of the hazard occurring
Impact – the extent of the impact in terms of people affected

Scoring:

Severity
1 = slight (injuries requiring no more than first aid or brief absence from work)
2 = serious (injuries requiring medical treatment or more than three days off work)
3 = major (death or severe injury may result; this may be in the long term)

Probability
1 = low (event will seldom occur)
2 = medium (event will occur occasionally)
3 = high (event will occur frequently)

Impact
1 = individual (single individual affected)
2 = group (endangers individuals within the immediate vicinity)
3 = department (endangers individuals within a wide area)

NOTES ON REVIEWING ASSESSMENTS

If the assessment is unchanged then sign and date the existing form. If the assessment
has changed then it should be re-submitted for approval and all existing users must read
and sign again.
For high risk procedures (9+) the assessment should be reviewed annually.
