AFFIDAVIT OF INSERTION
I, DAISY BELLA M. CERO, of legal age, married to Simeon L. Cero, Jr.,
Filipino and a resident of Brgy. Poblacion, Alabel, Sarangani Province,
Philippines, after having first been duly sworn to in accordance with law, do
hereby depose and say that:

1. I am the DONEE in the Deed of Donation executed between me and Henry

M. Morales with TCT No. T-9547, notarized by Atty. Antonio D. Deleña as per
Doc. No. 36, Page No. 9, Book No. CLV, Series of 2019;

2. Unfortunately, my civil status as “married to Simeon L. Cero, Jr.” was

not declared in said document;

3. Through this Affidavit, I hereby declare under oath that my civil status is
“married to Simeon L. Cero, Jr.” and request such to be inserted or
declared in the abovementioned document; and

4. I am executing this Affidavit to attest to the truth of the foregoing facts for
all intents and purposes this may legally serve.

IN WITNESS WHEREOF, I have hereunto affixed my signature this 6th

day of March, 2019 at Glan, Sarangani Province, Philippines.
