Terms & Conditions:

(a) Eligible family members refer to spouse & children for married employees; and parents for single staff.
(b) Payment to be made upon registration on a first-come, first-served basis.
(c) Employee and accompanied guest(s) shall not hold the Company or the Recreation Committee responsible
in the event of any accident or injury arising from this event and shall indemnify others against any claims.
(d) Absenteeism on actual day without valid reason will result in a No-Show Fee imposed at full cost of $65 per
employee/accompanied guest.
(e) Pulling out after the registration date will result in a Pull-Out Fee imposed at full cost of $65 per
employee/accompanied guest.
(f) Please submit the registration form by 08 May 2019, Wednesday to

(g) The Recreation Committee reserves the right to cancel your registration and refund you, the payment in
full, if you are unable to participate in this event due to work exigencies and you are recalled to work on
the same day.
