Identifying Information

ABC is 14 years old girl. She is first born and has two younger brothers. Her father is
businessperson whereas mother is a homemaker. She is Muslim and belongs to an Urdu speaking
family with middle socioeconomic status. She is living in a joint family set up, residing in
Gulistan-e-Jauhar, Karachi.

Referral Source and Presenting Problems

ABC came with her parents to the Institute of Behavioral Sciences (IBS), for the purpose of
psychological assessment and psychotherapy. Her presenting problems as reported by her mother
include; illusions, fearfulness and irritability.

Tests Administered

(1) Human Figure Drawing (HFD)


(2) Thematic Apperception Test (TAT)
(3) Rorschach Inkblot Test (ROR)
(4) Positive and Negative Syndrome Scale (PANSS)

Behavior During Assessment Sessions

ABC was on time and properly dressed for the session. Throughout assessment session, she
appeared guarded. She hesitated to answer verbally. With motivation from the examiner, she
responded. Moreover, she appeared depressed and low.

Psychological Evaluation

Projective analysis reveals that ABC’s self-image is inadequate. Furthermore, some anxiety and
insecurity is also revealed. Further, she perceives her environment as hostile and threatening. Her
dominant needs are of aggression and acquisition. Undergratification of these needs give
significant conflicts of intra vs. extra aggression and id vs. superego. Her nature of anxieties is
related to physical harm and punishment. To deal with these anxieties and conflicts, she uses the
defense mechanism of acting out and regression.

Further, projective analysis reveals that ABC tends to overemphasize the obvious aspects of a
situation rather than integrating her environment. This makes her problem-solving skills poor.
Moreover, she is defensive and has an ability to deliberately suspend or control her affect.
However, she has a predictable, stereotyped manner of approaching the world due to fear of
making mistakes and a tendency to be overwhelmed in complex situations. Projective analysis
also reveals that she is immature and has a low sophistication level. Further, she has a high
aspirational level. However, she often underestimates the actual efforts required to accomplish
goals so she tends to expend less effort than needed or required to process information, which
makes her more prone to disappointments.

Scores on PANSS indicate her symptoms within the ‘Much Below Average’ Range indication of
psychosis.

Conclusion

After analyzing client’s history and test results, it is revealed that her stress levels are high and
she needs a break from responsibilities and pressure that might be making her feel overwhelmed.
Moreover, due to her good insight, she can recover with proper counseling.

Recommendations

 Cognitive-behavioral therapy will help client modify and correct her cognitive distortions
and self-defeating beliefs.
 Family Counseling will help the family to adopt productive and healthy behaviors that
will facilitate client’s recovery.
 Teaching productive coping strategies is very important to help her deal with stress that
will improve her mental and physical well-being.
