EMPLOYMENT CONTRACT

KNOWN ALL MEN BY THESE PRESENTS:

This agreement is entered between the ff: ________________________________of legal age,

married/single and a resident of Dingalan, Aurora referred to as the employer

-and-

_______________________________ of legal age, married/single and a resident of ________________

_______________________________ as the employee.

Parties have to an agreement in connection and the employment of Staff Nurse by the

________ under the ff. terms and conditions:

1. WORK CONDITION:

The employee hired as Staff Nurse.

2. JOB DESCRIPTION

As the Staff Nurse your task/job/responsibilities are as follows.

 Prepare and evaluates the nursing care plan for each patient.
 Assist in planning nursing care.
 To escort patient to the room/ward.
 Prepares and administers medication and treatments as prescribed by Physician.
 Assist in diagnostic and therapeutic assessments of patient’s condition.
 Initiates appropriate measures in emergency situations.
 Gives oral and written reports of pertinent information to incoming staff.
 Makes daily rounds, recording and reporting symptoms and progress.
 Responsible for safety and welfare of patients through proper implementation of nursing
procedures .Observe Rights of administration.
 Taking rounds of the patient at regular interval and fill up vital signs as per nursing sheet.
 Follows Universal precautions and Infection Control Guidelines
 Consistently demonstrates professional attitudes and manner including confidentiality.
 Maintains attendance and observes working hours ,starting time, lunch period, breaks,
etc
 Complies with work rules, regulations, and instructions, follows established system and
procedures, and completes assignments with minimal supervision.
 Admission and discharge of patients.
 Administration of medication and injections.
 Collection labelling and dispatching of specimens.

3. Status of Employment: The employee is hired as follows

a. Probationary Employee

b. Regular Employee

c. Contractual Employee

d. Seasonal Employee

4. Compensation:

The employee shall receive the salary of Php__________ (Payment of the salary shall be
twice a month. Usually 15th or 30th)

5. Work Schedule

The employee shall report to work 5 days a week, 12 hours and 2 days off.

6. Dismissal and other penalties: The employee is bound to follow the rules and regulations (manual)
of the employer/company. any infraction or violation shall be penalized as provided for its rules
and regulations. Employee cannot be dismissed or personalized and just authorized causes and
the observance of due process.

7. Non-Disclosure Clause.
The employee shall not divulge any matter relating to sald business of the employee or matters
relating to any costumer/client/patient and may become known to him by reason of his
employment or otherwise save in so far as may be necessary in the interest of the said business.

8. Effectivity

This contract is effective upon the signing of this contract and the submission of the documentary
requirements by the employer to the employee.
