PART I: THE STUDENT

Strength
 I have been receptive to the ideas presented by my teacher and my classmates
 I have exerted effort to meet the requirements in this subject/course.
 My expectations for his subject/course have been met.

Weaknesses
 I have participated actively in class discussion and activities.
 I have worked more conscientiously in this subject/course than in other subject/course.

PART II

Strength
 There were many opportunities for me to showcase my creativity
 This subject is relevant to my daily life
 I am fully satisfied with the way this subject/course was handle/conducted.
 This subject is worth taking.

Weaknesses
 The pacing of this subject is just right (not too fast, not too slow.
 A syllabus or a course outline was made available at the beginning of the school year.
 There have been departures from the syllabus but these departures were necessary and beneficial

 Conducts activity that prepares me enthusiastically for the lesson of the day.
 Speaks clearly so I can hear the instructions/directions for the tasks
 Helps me reflect implications and insights about the day’s lesson for me to do something that
will positively contribute to my personhood.
 Allows me to make conclusion or give “big ideas” about the learning experiences
 Conducts stimulating and powerful focusing activity that makes me interested to be attentive in
the class
 Goes around the classroom to make sure that I am on task and doing my work
 Check the the cleanliness of the classroom to make me feel happy with the environment I am in

Weaknesses
 Does the following routinary activities
 Set the mood to focus for he day’s lesson
 Checks who are and not in the class to make me feel that the teacher is concerned about
us.
 Sets the physical arrangement of chairs for the kind of activity set for the day to facilitate
the accomplishment of the task

Areas of Improvement
 Asks questions to the students and make them think deeply, explore connections with other
disciplines and relate life experiences to lesson.
