Sub: Dispensing with submission of test reporl anil proof of ownership
document for release of loads upto 20 kW for DS (Only
Residential) connections.

It has been
decided that submission of test report and proof of
ownership by domestic Supply applicants having load upto 20 kW is henceforth
dispensed with subject to the condition that the applicanuconsumer shall
undertake to the PSPCL as under:-

" That all the electrical installation work within his/their premises is as per
the provisions of Central Electricity Authority (Measures Relating to Safet,
and Electrical Supply) Reg-2010 and as per relevant standards of BlS.
Further, wiring has been carried out by an electrical contractor licensed in
this behalf by the covernment of Punjab and under the direct supervision
of a person holding a certificate of competency and by a person holding a
permit issued or recognized by the Government of Punjab and has

obtained a test report from electrical contractor in this regard and the Test
. Report is in his posgession.
That the applicant shall at all times be solely responsible for the
tnaintenance of his installation in such condition to be free from danger
That the applicant shall indemnify PSPCL against any loss to the applicant
accrued on this account."
The simplified A&A form with declarations regarding ownership of premises
and test report shall be applicable for release of domestic supply connections
(only residential) upto 20 kW as per attached performa.

This issues with the approval of competent authority.

Meticulous compliance of the above mav be ensured bv all the field
officers.

maintenaftee of my/our installation in such condition as to be free from
