PC-I

Proforma for Preparation of Projects


(For NEW Projects)
Review
As the name suggests PC-I form is a framework for new projects. This form consists of three
sections:

 Part A: Project Digest.


 Part B: Project Requirements
 Part C: Appraisal and Analysis

Part A: Project Digest


This section consists of Project Title, Location of Project, Responsible Authorities, Main
Objectives, Implementation, Cost of project and Financing. This is the main basis and formation
of the project.

Part B: Project Requirements


In this section a statement of work is required it describes the work requirements for a specific
project along with its performance and design expectations. The main purpose of this document
is to define the liabilities, responsibilities and work agreements between two parties, usually
clients and service providers.
The program evaluation and review technique (PERT) is also required it is a statistical tool used
in project management, which was designed to analyze and represent the tasks involved in
completing a given project.
Moreover, Project Staff is finalized which includes Professional and Technical, Administrative,
Executive and managerial, Clerical, Skilled, Unskilled and as well as consultants from local or
foreign sources.

Part C Appraisal and Analysis


Appraisal is assessing the viability of a project or proposal. It includes calculating the feasibility of the
project before committing resources to it. Companies use this method to choose the best project that
would help them to attain their goal.

This section requires info about project that is it beneficial? In terms of financial, economic,
social or others.
Financial Analysis, Economic Analysis and Sensitivity and Risk Analysis.
This section also requires different financial ratio and demands it to be beneficial for project
approval.

PC-II
Proforma for Preparation of Proposal for
Conducting Feasibility Study/Survey
PC-II is about survey, feasibility and terms of reference report. A feasibility report is a business
and strategic plan that steers a business into the right direction towards success.
A survey about the project is always conducted to make sure that whether the project will be
successful in said so environment.
The terms of reference (ToR) document defines all aspects of how a consultant or a team will
conduct an evaluation. It defines the objectives and the scope of the evaluation, outlines the
responsibilities of the consultant or team, and provides a clear description of the resources
available to conduct the study. Developing an accurate and well specified. ToR is a critical step
in managing a high-quality evaluation. The evaluation ToR document serves as the basis for a
contractual arrangement with one or more evaluators and sets the parameters against which the
success of the assignment can be measured.

PC – III
Quarterly Progress Report/ Financial REPORT
This Performa has two sections one is for companies to submit quarterly progress reports which
indicates the approval of capital cost as well as how cash was used in the day to day operations
of the project. The second section requires financial and physical progress from companies
which is to be submitted on monthly basis this includes expenditure and physical achievements
of the month.

PC-IV
Project Completion Report
As the name suggests this is the final stage of the project, it is mandatory to be created on
completion of the project. This report requires the following to be included: the physical targets
and achievements, plan and actual expenditure, achievement of objectives, revenue from
services/revenue generation, Mechanism for Sustainability of Project/Activities After

Completion, Quantifiable Benefits achieved, Financial/Economic Results Based on Actual Cost,
Lessons learned during project implementation, Suggestions for planning & Implementation of
Similar Projects.

PC-V
ANNUAL PERFORMANCE REPORT AFTER
COMPLETION OF PROJECT
PC-V is annual operational report which has to be submitted to Planning and Development
Department after completion of the project for consecutive five years. The following are required
for this report: Planned and actual recurring cost of the project, with Details Planned & actual
manpower employee, planned and actual physical output of the project, planned and actual
income of the project, planned and actual benefits to the economy planned and actual social
benefits, planned and actual cost per unit produced/sold, marketing mechanism arrangement for
maintenance of building & equipment.
