INTRODUCTION
Health care (or healthcare) is the diagnosis, treatment, and prevention of disease, illness,
injury, and other physical and mental impairments in humans. Health care is delivered by
practitioners in medicine, optometry, dentistry, nursing, pharmacy, allied health, and other
care providers. It refers to the work done in providing primary care, secondary care, and
tertiary care, as well as in public health.
Access to health care varies across countries, groups and individuals, largely influenced by
social and economic conditions as well as the health policies in place. Countries and
jurisdictions have different policies and plans in relation to the personal and population-based
health care goals within their societies. Health care systems are organizations established to
meet the health needs of target populations. Their exact configuration varies from country to
country. In some countries and jurisdictions, health care planning is distributed among
market participants, whereas in others planning is made more centrally among governments
or other coordinating bodies. In all cases, according to the World Health Organization
(WHO), a well-functioning health care system requires a robust financing mechanism; a
well-trained and adequately-paid workforce; reliable information on which to base decisions
and policies; and well maintained facilities and logistics to deliver quality medicines and
technologies.
Health care can form a significant part of a country's economy. In 2008, the health care
industry consumed an average of 9.0 percent of the gross domestic product (GDP) across the
most developed OECD countries. The United States (16.0%), France (11.2%), and
Switzerland (10.7%) were the top three spenders, but life expectancy was highest in Japan
(82.6), Australia (81.4), and Canada (81.4). (See international comparisons.)
Health care is conventionally regarded as an important determinant in promoting the general

health and well-being of people around the world. An example of this is the worldwide
eradication of smallpox in 1980—declared by the WHO as the first disease in human history
to be completely eliminated by deliberate health care interventions.
Health care delivery
The delivery of modern health care depends on groups of trained professionals and
paraprofessionals coming together as interdisciplinary teams.[4][5] This includes professionals
in medicine, nursing, dentistry and allied health, plus many others such as public health
practitioners, community health workers and assistive personnel, who systematically provide
personal and population-based preventive, curative and rehabilitative care services.

While the definitions of the various types of health care vary depending on the different
cultural, political, organizational and disciplinary perspectives, there appears to be some
consensus that primary care constitutes the first element of a continuing health care process,
that may also include the provision of secondary and tertiary levels of care. Healthcare can be
defined as either public or private.
Health care administration and regulation
The management and administration of health care is another sector vital to the delivery of
health care services. In particular, the practice of health professionals and operation of health
care institutions is typically regulated by national or state/provincial authorities through
appropriate regulatory bodies for purposes of quality assurance. Most countries have
credentialing staff in regulatory boards or health departments who document the certification
or licensing of health workers and their work history. New Obama care requires all
individuals to have some kind of health benefits. It include: mental health services,
Emergency services, prescription drug coverage, pediatric services, Rehabilitation services,
laboratory services and prenatal care. There four type of plans that cover same service but are
different in cost; Bronze covers 60% of medical costs, Sliver covers 70% medical costs, Gold
covers 80% medical costs and Platinum covers 90% of medical costs. Employers have to
provide healthcare insurance to employees who work full time and if not they have to pay a
plenty fee. People who are unemployed or self-employed have a new option to acquire
healthcare from new state-based exchanges marketplaces. Premium of the insurance will be
only 9.5% of the household income. If someone don't qualify for Medicaid and makes 15,856
as a single adult, they will have no financial help. Because the Tax credits only help to pay
insurance for people with income of 100 and living under 400% federal poverty level. This
problem would be solved through future legislations.
Health information technology
Health information technology (HIT) is “the application of information processing involving

both computer hardware and software that deals with the storage, retrieval, sharing, and use
of health care information, data, and knowledge for communication and decision making”
(Brailer, & Thompson, 2004). Technology is a broad concept that deals with a species' usage
and knowledge of tools and crafts, and how it affects a species' ability to control and adapt to
its environment. However, a strict definition is elusive; "technology" can refer to material
objects of use to humanity, such as machines, hardware or utensils, but can also encompass
broader themes, including systems, methods of organization, and techniques. For HIT,
technology represents computers and communications attributes that can be networked to
build systems for moving health information. Informatics is yet another integral aspect of
HIT.

INTRODUCTION HTML
HyperText Markup Language (HTML) is the main markup language for creating web
pages and other information that can be displayed in a web browser.
HTML is written in the form of HTML elements consisting of tags enclosed in angle brackets
(like <html>), within the web page content. HTML tags most commonly come in pairs like
<h1> and </h1>, although some tags represent empty elements and so are unpaired, for
example <img>. The first tag in a pair is the start tag, and the second tag is the end tag (they
are also called opening tags and closing tags). In between these tags web designers can add
text, further tags, comments and other types of text-based content.
The purpose of a web browser is to read HTML documents and compose them into visible or
audible web pages. The browser does not display the HTML tags, but uses the tags to
interpret the content of the page.
HTML elements form the building blocks of all websites. HTML allows images and objects
to be embedded and can be used to create interactive forms. It provides a means to create
structured documents by denoting structural semantics for text such as headings, paragraphs,
lists, links, quotes and other items. It can embed scripts written in languages such as
JavaScript which affect the behavior of HTML web pages.
Web browsers can also refer to Cascading Style Sheets (CSS) to define the appearance and
layout of text and other material. The W3C, maintainer of both the HTML and the CSS
standards, encourages the use of CSS over explicit presentational HTML.

History

Development
In 1980, physicist Tim Berners-Lee, who was a contractor at CERN, proposed and
prototyped ENQUIRE, a system for CERN researchers to use and share documents. In 1989,
Berners-Lee wrote a memo proposing an Internet-based hypertext system.[2] Berners-Lee
specified HTML and wrote the browser and server software in the last part of 1990. In that
year, Berners-Lee and CERN data systems engineer Robert Cailliau collaborated on a joint
request for funding, but the project was not formally adopted by CERN. In his personal
notes[3] from 1990 he listed "some of the many areas in which hypertext is used" and put an
encyclopedia first.
The first publicly available description of HTML was a document called "HTML Tags", first
mentioned on the Internet by Berners-Lee in late 1991.[5][6] It describes 18 elements
comprising the initial, relatively simple design of HTML. Except for the hyperlink tag, these
were strongly influenced by SGMLguid, an in-house SGML-based documentation format at
CERN. Eleven of these elements still exist in HTML 4.
HyperText Markup Language is a markup language that web browsers use to interpret and
compose text, images and other material into visual or audible web pages. Default
characteristics for every item of HTML markup are defined in the browser, and these
characteristics can be altered or enhanced by the web page designer's additional use of CSS.
Many of the text elements are found in the 1988 ISO technical report TR 9537 Techniques
for using SGML, which in turn covers the features of early text formatting languages such as
that used by the RUNOFF command developed in the early 1960s for the CTSS (Compatible
Time-Sharing System) operating system: these formatting commands were derived from the
commands used by typesetters to manually format documents. However, the SGML concept
of generalized markup is based on elements (nested annotated ranges with attributes) rather
than merely print effects, with also the separation of structure and markup; HTML has been
progressively moved in this direction with CSS.
Berners-Lee considered HTML to be an application of SGML. It was formally defined as

such by the Internet Engineering Task Force (IETF) with the mid-1993 publication of the

first proposal for an HTML specification: "Hypertext Markup Language (HTML)" Internet-
Draft by Berners-Lee and Dan Connolly, which included an SGML Document Type
Definition to define the grammar.[8] The draft expired after six months, but was notable for its
acknowledgment of the NCSA Mosaic browser's custom tag for embedding in-line images,
reflecting the IETF's philosophy of basing standards on successful prototypes. [9] Similarly,
Dave Raggett's competing Internet-Draft, "HTML+ (Hypertext Markup Format)", from late
1993, suggested standardizing already-implemented features like tables and fill-out forms.
After the HTML and HTML+ drafts expired in early 1994, the IETF created an HTML
Working Group, which in 1995 completed "HTML 2.0", the first HTML specification
intended to be treated as a standard against which future implementations should be based.
Further development under the auspices of the IETF was stalled by competing interests.
Since 1996, the HTML specifications have been maintained, with input from commercial
software vendors, by the World Wide Web Consortium (W3C). However, in 2000, HTML
also became an international standard (ISO/IEC 15445:2000). HTML 4.01 was published in
late 1999, with further errata published through 2001. In 2004 development began on
HTML5 in the Web Hypertext Application Technology Working Group (WHATWG), which
became a joint deliverable with the W3C in 2008.
Elements
HTML documents are composed entirely of HTML elements that, in their most general form
have three components: a pair of tags, a "start tag" and "end tag"; some attributes within the
start tag; and finally, any textual and graphical content between the start and end tags,
perhaps including other nested elements. The HTML element is everything between and
including the start and end tags. Each tag is enclosed in angle brackets.
The general form of an HTML element is therefore: <tag attribute1="value1"

attribute2="value2">content</tag>. Some HTML elements are defined as empty elements
and take the form <tag attribute1="value1" attribute2="value2" >. Empty elements may
enclose no content, for instance, the BR tag or the inline IMG tag. The name of an HTML
element is the name used in the tags. Note that the end tag's name is preceded by a slash
character, "/", and that in empty elements the end tag is neither required nor allowed. If
attributes are not mentioned, default values are used in each case.

content and it may not have an end tag.

Comments can help in the understanding of the markup and do not display in the webpage.
There are several types of markup elements used in HTML:
Structural markup describes the purpose of text
For example, <h2>Golf</h2> establishes "Golf" as a second-level heading. Structural

markup does not denote any specific rendering, but most web browsers have default styles
for element formatting. Content may be further styled using Cascading Style Sheets (CSS).

Presentational markup describes the appearance of the text, regardless of its purpose
For example <b>boldface</b> indicates that visual output devices should render
"boldface" in bold text, but gives little indication what devices that are unable to do this (such
as aural devices that read the text aloud) should do. In the case of both <b>bold</b> and
<i>italic</i>, there are other elements that may have equivalent visual renderings but which
are more semantic in nature, such as <strong>strong text</strong> and <em>emphasised
text</em> respectively. It is easier to see how an aural user agent should interpret the latter
two elements. However, they are not equivalent to their presentational counterparts: it would
be undesirable for a screen-reader to emphasize the name of a book, for instance, but on a
screen such a name would be italicized. Most presentational markup elements have become
deprecated under the HTML 4.0 specification in favor of using CSS for styling.
HTTP
The World Wide Web is composed primarily of HTML documents transmitted from web
servers to web browsers using the Hypertext Transfer Protocol (HTTP). However, HTTP is
used to serve images, sound, and other content, in addition to HTML. To allow the web
browser to know how to handle each document it receives, other information is transmitted
along with the document. This meta data usually includes the MIME type (e.g. text/html or
application/xhtml+xml) and the character encoding (see Character encoding in HTML).
In modern browsers, the MIME type that is sent with the HTML document may affect how
the document is initially interpreted. A document sent with the XHTML MIME type is
expected to be well-formed XML; syntax errors may cause the browser to fail to render it.
The same document sent with the HTML MIME type might be displayed successfully, since
some browsers are more lenient with HTML.
The W3C recommendations state that XHTML 1.0 documents that follow guidelines set forth
in the recommendation's Appendix C may be labeled with either MIME Type. [58] XHTML 1.1
also states that XHTML 1.1 documents should[59] be labeled with either MIME type.
HTML e-mail
Most graphical email clients allow the use of a subset of HTML (often ill-defined) to provide
formatting and semantic markup not available with plain text. This may include typographic
information like coloured headings, emphasized and quoted text, inline images and diagrams.
Many such clients include both a GUI editor for composing HTML e-mail messages and a
rendering engine for displaying them. Use of HTML in e-mail is controversial because of
compatibility issues, because it can help disguise phishing attacks, because of accessibility
issues for blind or visually impaired people, because it can confuse spam filters and because
the message size is larger than plain text.

Naming conventions
The most common filename extension for files containing HTML is .html. A common
abbreviation of this is .htm, which originated because some early operating systems and file
systems, such as DOS and the limitations imposed by FAT data structure,[61] limited file
extensions to three letters.
HTML Application
An HTML Application (HTA; file extension ".hta") is a Microsoft Windows application that
uses HTML and Dynamic HTML in a browser to provide the application's graphical
interface. A regular HTML file is confined to the security model of the web browser's
security, communicating only to web servers and manipulating only webpage objects and site
cookies. An HTA runs as a fully trusted application and therefore has more privileges, like
creation/editing/removal of files and Windows Registry entries. Because they operate outside
the browser's security model, HTAs cannot be executed via HTTP, but must be downloaded
(just like an EXE file) and executed from local file system .
Cascading Style Sheets
Cascading Style Sheets (CSS) is a style sheet language used for describing the presentation
semantics (the look and formatting) of a document written in a markup language. Its most
common application is to style web pages written in HTML and XHTML, but the language
can also be applied to any kind of XML document, including plain XML, SVG and XUL.
CSS is designed primarily to enable the separation of document content (written in HTML or
a similar markup language) from document presentation, including elements such as the
layout, colors, and fonts. This separation can improve content accessibility, provide more
flexibility and control in the specification of presentation characteristics, enable multiple
pages to share formatting, and reduce complexity and repetition in the structural content
(such as by allowing for tableless web design). CSS can also allow the same markup page to
be presented in different styles for different rendering methods, such as on-screen, in print,
by voice (when read out by a speech-based browser or screen reader) and on Braille-based,
tactile devices. It can also be used to allow the web page to display differently depending on
the screen size or device on which it is being viewed. While the author of a document
typically links that document to a CSS file, readers can use a different style sheet, perhaps
one on their own computer, to override the one the author has specified.
CSS specifies a priority scheme to determine which style rules apply if more than one rule
matches against a particular element. In this so-called cascade, priorities or weights are
calculated and assigned to rules, so that the results are predictable.

The CSS specifications are maintained by the World Wide Web Consortium (W3C). Internet
media type (MIME type) text/css is registered for use with CSS by RFC 2318 (March 1998),
and they also operate a free CSS validation service.
Syntax
CSS has a simple syntax and uses a number of English keywords to specify the names of
various style properties.
A style sheet consists of a list of rules. Each rule or rule-set consists of one or more selectors,
and a declaration block.
