28, 2019

Thursday means we’re staying at school and have our regular schedule. We need
to finish and pass all requirements and paper works to our clinical instructor. During the
morning, we helped to fix the nutrition room as our assignment for our duty in Medical
Ward.

Today we had our duty inside the classroom. My group has been tasked to take
charge of fixing the chn room. Unfortunately for us, me along with my 2 other group mates,
we had to go out and take our exam at Merge Morayta. We got back after lunch and went
to accomplish.

On the 3rd day we stayed in classroom to fix some requirements in head nursing.
But me, Ms. Onyeji, Ms. Wilson and Torreja ask permission to Maam Merin to have our
prelim exam on Merge Review center for our Competency Appraisal subject.

Today we supposed to have a quiz regarding on what we do and what we have


learned in our duty. We leave early to take our prelim examination. We have to leave
early because we need to be back before 12 noon. She was late due to traffic. So, we
don’t have a choice but to wait for her.

REFLECTIONS

February 26, 2019

As part of our RLE requirements we are assigned to have our duty in a Barangay
Health Center particularly in Dapitan Health Center. We had our courtesy call to the
Belmonte Health Center first before we proceed to our duty in health center. It was my
first time to have duty in Dapitan Health Center that’s why I listened ver well about their
resulations and policies.

Our group were assigned in the treatment room. I can remember giving my first
injection like it was yesterday. My hands were shaking and I could feel the beads of sweat
on my forehead. Of course, my patient was so afraid of getting an injection there were
tears in his/her eyes. My professor was standing right over my shoulder. I have learned
the importance to not be afraid to ask when you are unsure of something since our clinical
instructor are good resources for knowledge. Knowing exactly what you have to do before
to the patient’s will boost our confidence. For me it’s important that after asking questions
and practicing over and over, be confident in yourself.

February 27, 2019

Second day of our duty, our group assigned in admission area. Subjective and
objective data collection are an integral part of this process. Part of the assessment
includes data collection by obtaining vital signs such as temperature, respiratory rate,
heart rate, blood pressure, and pain level using an age or condition. The assessment
identifies current and future care needs of the patient by allowing the formation of a
nursing diagnosis. Having duty in this area for me is not so easy because there’s a lot of
papers to do in admitting but it helped me to recognizes normal and abnormal patient
physiology and prioritize interventions and care that will be given to patients.
February 28, 2019

During the morning, our previous clinical instructor gave us a task to finish the
consultation room and turned it to nutrition room. As a head nurse for the day I was the
one who is responsible to assigned my staff nurses to specific tasks.

March 4, 2019

On this day, we’re assigned to dispensing area. I don’t feel nervous because I
asked the person who have done having duty in dispensing area for me to know further
instructions needed in giving medications to the patients. I liked in dispensing area than
the other area because there’s a lot of papers to do in admitting and treatment area. I
enjoyed having duty here event if we only have few patient since scheduled day is every
Tuesday and Thursday. We instructed them to come back on the specific date that was
given to them.

March 5, 2019

On this day we just admitted few patients because we had a home visit for our
case management regarding non communicable diseases such as hypertension and
diabetes that will be going to present to our clinical instructor. The barangay officials
helped us to find residents with the said disease, for us to collect data and to make an
interview about their condition. It’s not easy to get patient in the barangay since other
residents is so busy doing other things like going to work and doing household chores. I
have learned the importance of having rapport and therapeutic relationship to our patient
to gain their trust and for them to cooperate with us.
After our duty in Dapitan Health Center we went back to school earlier and stayed
in the CHN simulation room to have meeting about health teaching about expanded
program for immunization that will be discuss to health center on the next day.

March 6, 2019

On the last day and final duty for head nursing in community health nursing. We
had our health teaching about expanded program for immunization. We have discussed
that immunization is a proven tool for controlling and even eradicating infectious
diseases.

I’m so glad because the patient’s in health center were cooperative and asked
some questions about immunization. Since it was our last day in the community we
bought foods for the Health Center and ate together.

NUTRITION ROOM

Our previous clinical instructor gave us a task to finish the consultation room and
turn it to nutrition room. As a head nurse for the day I was the one who is responsible to
assigned my staff nurses to specific tasks. I assigned Mandap and Manlapaz to carry
heavy stuff from consultation room to storage room. Malayao was the one who make sure
the consultation was clean and tidy. Pangilinan and I were assigned to arrange the
posters according to childhood to adulthood. Sorongon and Solmayor were assigned to
fix the kitchen area and put the plates and glasses in place. Lastly, Verdejo and Onyegi
were assigned to fix the chairs. We must know how important nutrition for us. Every
student has the potential to do well in school. Taking action today to provide healthier
choices in schools can help to set students up for a successful future full of possibilities.
