Opening of Bid Nos.: 50-19-03-001, 50-19-03-002, 50-19-03-003, 50-19-03-004, 50-19-03-005

The opening and Awarding of bids for the above enumerated Bid Nos. was presided by the
Chairperson Mrs. Profetisa P. Castillo. The presider instructed Mrs. Gina Mary Bandigan to lead the
prayer before the formal opening.

The Chairperson officially opened the activity at 1:26pm instructing the Secretary Mr. Fred
Ryan C. Deaño for the Attendance and Call to Order.

The Secretary made the roll call of the members of the BAC including the ADAS II – Mrs.
Vilma Y. Pichon, School Physical Facilities- Mr. Gerson B. Lasmarias and the School Nurse- Ms.
Emerald N. Ocon.

After the roll call of attendance, the Chairperson instructed Mrs. Ginalyn G. Batitang to open
the Bidding Envelope accompanied by Mr. Gerson Lasmarias and ms. Emerald N. Ocon.

Mrs. Ana C. Peliña moved for the checking of the Bids submitted for Eligibility seconded by
Mrs. Vilma Pichon.

The members of the BAC checked all the Bids submitted and made the following declaration.

The Chairperson solicited any motion for the Eligibility checked.

 Mrs. Ana C. Peliña moved that all bids submitted are eligible seconded by Mrs. Gina Mary
Bandigan.

The Chairperson solicited any motion for the awarding.

Mrs. Vilma Pichon moved for awarding of the Bids by selecting and encircling the price of
the lowest calculated responsive bid seconded by Ms. Emerald N. Ocon and Gerson B. Lasmarias.

The Chairperson instructed the BAC Secretary to assist committee in the Awarding of Bids
for Bid No. 50-19-03-001, 50-19-03-002, 50-19-03-003, 50-19-03-004, 50-19-03-005 by selecting
and encircling the price of the lowest calculated.

The Secretary informed the Chair that all lowest calculated responsive bids have been
selected.

The Chairperson instructed the Secretary to prepare the Abstract of Bids with corresponding
quotations of the eligible bidders and encircling the price of the lowest calculated responsive bid
which will be signed by all the members present including the witness to be forwarded to the School
Head for Approval.

The Chairperson also instructed the Secretary to prepare the minutes of the Opening and
Awarding which will be signed by all members present including the witness/ observer/s and copy
thereof be send to the Commission on Audit.

After everything has been put to order, Mrs. Ana C. Peliña raised that she has been a member
of the BAC for years and she wanted to relinquish her position to other Master Teachers. The body
agreed to her decision. It was put into motion by Mrs. Gina Mary Bandigan and seconded by Mrs.
Ginalyn G. Batitang.

The Chairperson officially closed the Opening and Awarding at 2:00pm.
