Dear Doctor,

Re: Ms. Gwen Watego, DOB: 25/03/1972

I am writing to refer this patient, an Aboriginal widow who requires further investigation
for possible diabetes milletus 2.

In regards to her health history, she had pneumonia a year ago. She is unsure about
her family medical history as she does not know about her parents. Generally, she is
overweight and does minimal exercise. She smokes 15 cigarettes daily and a moderate
drinker. She has no known allergies as well as no current medication.

Ms. Watego was presented in the accident and emergency department following a fall
outside the shopping centre. Fortunately, there is no LOC and head injury. She
sustained grazed on both palms in which they were cleansed with antiseptic.

Whilst in the hospital, she complains of pain on her feet. On examination, the left and
right plantar surface have ulcer and callus respectively. Random glucose test was done
which showed 11 mmol/L indicating the possibility of diabetes. Risks and benefits of
quitting smoking has been tackled, and she has been referred to Quitline.

I would be grateful if you will furtherly assess and manage this patient.
