QA/QC Weekly Meeting Minutes No. 08
NCR: BSBG noted and tabled the pending NCRs which are open either due to no proposed
corrective action given or the corrective actions have been agreed with BSBG but the
implementation at site is still incomplete. Refer to NCR register.
SOR: BSBG noted that a total of 21 Nos. of SOR are open since September 2018 up until December
2018. Most of the SORs were responded by AFC with corrective action and were returned by BSBG
with noted acceptance. From thereafter, no further responses were made on records. Refer to
attached SOR register. BSBG noted that the escalation from SOR to NCR is mandatory, due to failure
to respond and close the related SORs as agreed in the last meeting.

BSBG noted that below issues which are again prevalent during the site walk.
12. Secondary steel member for MEP supports commenced without approved shop
drawing, MIR for steel members, Calculations etc.
13. Reinforcement steel stacking in Zone 5
Parallel to above item 2.1, BSBG noticed that target dates tabled below by AFC are getting revised
and extended on similar account in respect of the first meeting. The target dates proposed were
missed subsequently hence are revised and extended by AFC whose new target dates are given
below heretofore. BSBG remarked that delay of rectification and response is unacceptable and
must be sorted out immediately pursuant to the contractual obligations.
The site walk was intended to assist and provide the site team proactive measures to avert
any recurrent issues thus diminishing the issuance of further NCRs and SORs of persistent
nature but will not be effective unless otherwise their involvement is fully materialized. AFC
have noted that internal instructions were given, and all PM’s are now required to attend
the site walks from next week onwards.
8.0 AOB
Intention is that future meetings shall discuss site wide quality matters and critical NCR.
