the source of water is situated, submit either of the following:
a) Private property: a right to use of water source duly notarized instrument,
executed by legitimate property owner in favor of water user/authorized WPA applicant.
b) Government property/Public lands -
Foreshore- MLA/FLA
Timberland - SLUP/FLAG
Mineral lands- MPSA
Protected Area - clearance from PAMB or Special Use Agreements in Protected Areas
LGU Property - Sanggunian Resolution

8. Well Drilling Data ( in case of existing groundwater source authenticated by the
i.e electric log, strata log, well design and pumping test or Actual minimum mean
discharge of the spring (for spring source)
9. Certificate of Potability from local health office (including Physical and Chemical
Analysis of Water for existing water source) if applicable:
10. ECC exscept for refilling stations; and
11. Geo-tagged photo of the water source
