Research Consent Form

You are invited to take part in a research study entitled “Leading from the Constituency: The
Student’s Voice” that is focused on the experiences that students have at Methodist University.
The researchers are inviting current students at Methodist University to participate in the study.
The researchers contacted you personally on the Methodist University campus. This form is part
of a process called “informed consent” to allow you to understand this study before deciding
whether to take part.

This study is being conducted by a research team of students who are in the Habitudes
Leadership Seminar Series. The researchers are: Jeremiah Wolbers, Erin Johnson, Kyle Johnson,
and Bryson Pinkney. The faculty/staff advisor is Dr. Eugenia J. Johnson, who is a facilitator of
the Habitudes program. You might also know Dr. Eugenia J. Johnson as the Director of
Academic Advising at Methodist University, but this study is separate from that role.

Background Information:

This study is designed to address a gap in the literature that exists regarding the successful and
unsuccessful experiences of students at Methodist University in Fayetteville, North Carolina.

Procedures:

If you agree to be in this study, you will be asked to:

 spend approximately 5 minutes to complete an online survey.


 complete the online survey only one time.

Voluntary Nature of the Study:

This study is voluntary. You are free to accept or turn down the invitation. No one at Methodist
University will treat you differently if you decide not to be in the study. If you decide to be in the
study now, you can still change your mind later. You may stop at any time.

Risks and Benefits of Being in the Study:

Being in this type of study involves some risk of the minor discomforts that can be encountered
in daily life, such as minor fatigue from completing the survey. Being in this study would not
pose risk to your safety or wellbeing.

Payment:

Students who participate have the opportunity to enter into a raffle for “Monarch Dollars” if they
so choose. The student must enter his/her student email address at the end of the survey in order
to enter into the raffle. The student email addresses will go into a different data collection area,
and will not be associated with any answers given in the survey.

Privacy:

Reports coming out of this study will not share the identities of individual participants. Details
that might identify participants, such as the location of the study, also will not be shared. Even
the researcher will not know who you are. This is an anonymous study in which no one knows
who participated, with consent implied through completion of the survey and no consent
signature. The researcher will not use your personal information for any purpose outside of this
research project. Data will be kept secure by password protection, and data encryption. Data will
be kept for a period of at least 5 years, as required by the university.

want to talk privately about your rights as a participant, you can call the Methodist University
Institutional Review Board at (910) 480-8494. Methodist University’s approval letter for this
study is attached and the approval expires on May 13, 2019.

Dr. Johnson,

I am happy to inform you that your project, Leading from the Constituency: the Students’
Voice, has been approved by the Methodist University Institutional Review Board. This project
approval is for the span of days beginning March 13, 2019 and ending on May 13, 2019. Per
Methodist University Institutional Review Board Policy, if you need to make any changes to the
protocol/project, you must inform the MUIRB chair and obtain MUIRB approval. In addition, if
there are any unanticipated events for the data you must also inform the IRB. If necessary, you
may apply for continuing authorization for your project and this must be done prior to the
current project's expiration date if you choose to extend the time necessary for you to
complete your research.
