Subject: Cradle inspection Report

With regard to the check list submitted by M/S ALUMACO you are further
advised upon inspection of cradle used by ALUMACO for glazing work at Hospital
Building. It is observed that there is no vertical life line fixed to the cradle. This life
line to be fixed for use of your workers to secure their safety harness belt while
working at height. In case failure of cradle, this life line will act as a fall arrest
equipment. As an immediate measure life line should be provided to reduce the
hazardous condition of glazing work.
Therefore you are instructed to take immediate necessary action to
provide the vertical life line to reduce the hazard.
