Notes: When we first arrived to the home visit Gabby was upset and was teary eyed. As soon as we brought out our activities, she became more engaged and playful. Other than

that, today was a typical day nothing out of the ordinary. Mom was not there during the intervention session but nurse was there.
Impression: I thought that the beginning of the intervention was good because Gabby was kneeling and reaching up and putting the animals in the
mouth. I thought that it was too easy and so I moved the bag higher but she wasn’t able to reach it so I helped her after she reached to lift her up to
place the animal in the mouth. I think it would have been better to keep it at the second level where she was strengthening her leg and getting that
practice and endurance of reaching. I feel that consistency and practice is better at first and building the endurance.

Objective Description of Data and Observations: Gabby asked who it was on the bag. Interventionist responded, “it’s an old lady”. Interventionist
showed Gabby “there was an old lady who swallowed the fly” pieces and Gabby pointed to the old lady’s mouth. When given an animal she
reached her arm and placed it into the old lady’s mouth. As the interventionist read the story when she came to the different animal parts, she
handed Gabby the animal and Gabby took the animal extended her arm and placed the animals in the mouth cut out in the paper bag. After two
animals the interventionist took the bag and placed it on a little bit higher surface so Gabby would have to reach a little further. Gabby scooted on
bottom using her hands. Gabby reached up towards the mouth but could not quite reach it. Interventionist helped Gabby move her leg behind her
to kneel and reach up. Gabby would reach up but not quite reach it. Gabby would reach up but was not able to get up to put the animals in the
mouth. The interventionist would physically assist Gabby to put the animals in the mouth.

Assessment and Evaluation: The first portion I had the bag on the floor and Gabby was would not kneel because she could put the animals in
without reaching or kneeling. After the second animal I moved the bag up to a chair. It was a little tougher but when Gabby’s leg was in a
kneeling position she could hold on to the edge of the chair and reach up and place the animals in the mouth. This is where I should have left it but
I thought it was too easy but looking back it wasn’t. I decided to move it on the television stand which the chair height as half the size. Gabby was
kneeling and reaching but she could not reach the mouth and put the animal in. I decided to put her on my knee but have her leg still in the
kneeling position. She would reach up and put the animal in. I realized that the goal was not to put the animal in but to kneel and reach was more
important.

Plan: For the next session our target is to kneel and reach up to gain strength in her leg. The activity that we will use is my “There Was An Old
Lady Who Swallowed a Fly” activity. I have put the old lady’s face on a large paper bag, with a hole cut out for the mouth and then I have animals
that go with the story to put in the mouth. Gabby will have the opportunity to kneel and reach up to put animals from the story in the old lady’s
mouth. The prompting I will use in the next session will be indirect verbal. I will read the story and when I read the animal Gabby can put the
animal in the mouth. I will use verbal promptings such as, “kneel”, “put your leg behind you if your crawling”, and “reach up and put the animal
in the old lady’s mouth”. I may also use physical prompts such as helping Gabby move her leg behind her in a kneeling position and holding her
in the kneeling position.
