The Avery Dennison InfoChain Express 3.0 solution configuration is installed using
3 fundamental components per installation.

1. SITE STATUS QUALITY – to ensure a stable install environment

2. PC REQUIREMENT - to ensure minimum needs for PRINT/SCAN&PACK
3. INTERNET REQUIREMENTS – to validate minimum bandwidth needs

This survey is the form in which factory environment information is captured.

The information captured is geared toward providing an improved understanding of
the technical installation environment and focused to assist in a successful on-site
installation of the ICE 3.0 system configuration needs.

It is important that extra care be used in filling out this form. It is more important to
note: HARDWARE CONFIGURATION installation can only commence when information provided is
verified. If, for any reasons, the conditions of the factory are different from the information provided
on this form and the factory conditions are found to be unfit for the installation to take place (e.g.
leakage of electricity, poorly configured internet connection), it cannot be successfully carried out.
Any re-visit to perform installation will be charged at the purchasers’ expense.

If at any time during the course of the interview or filling out the information required
below, there is difficulty with a response, please contact your Avery Dennison
technical support team immediately to review. When the form is completed, please
return it to your Avery Dennison technical support contact to schedule the ICE 3.0
installation.
