NARATIVE
RECKLESS IMPRUDENCE RESULTING IN PHYSICAL INJURIES AND DAMAGE TO PROPERTY
On June 4, 2016 at about 8:20 in the evening, a vehicular accident transpired along Maharlika Highway, infront of Caltex Gasoline Station, Sitio Pulo, Brgy. Caalibangbangan,
Cabanatuan City involving:
Vehicle 1 - Isuzu Tractor Head, colored white/green, bearing license plate number RGG 130 with highbed trailer, colored green, with plate number BUE 324, owned by Manuel and
Grace Dirige of Santiago Isabela, driven by MARVIN DE GUZMAN y INALDO, 26 years old, married, resident of Ramon Isabela, holder of Professional Driver’s License with number B12-
10-000176 valid until (2018-01-22); and
Vehicle 2 - Kawasaki Bajaj CT100 motorized tricycle, colored black, without plate number, with body number EG 276, driven by DEXTER TIONGSON y SANTOS, 35 years old, married,
resident of 3rd Avenue, Bernardo District, Cabanatuan City, no OR/CR and Driver’s License presented during investigation.
Initial investigation disclosed that VEH1 was temporary parked at the outer lane of the road facing South direction, when it was accidentally hit from behind by VEH2 while the latter
running heading towards same direction, as a result, VEH1 incurred damage on its left tail light, while VEH2 incurred heavily damages on its front portion both of still undetermined
amount. The driven sustained injuries on his forehead was brought to Dr. PJG Hospital for medical treatment. It has been observed that the driver of VEH2 is believed to be under the
influence of liquor during the incident.
However, after confrontation made to this Office by the parties concerned with regards to this incident, they agreed for the settlement of this case. The Officer-on Case is PO3 Dennis

On this time and date, duo ERWIN TIONGSON and MARVIN DE GUZMAN appeared before this office and requested to put on record that ERWIN TIONGSON stand as representative of
his brother DEXTER TIONGSON received the amount of Php.5,000.00 from MARVIN DE GUZMAN intended for financial assistance for medical expenses of DEXTER TIONGSON. In this
regard, both parties manifested their non-intent to pursue this case against each other. Hence, this case was settled amicably and is considered dropped and closed.
NARATIVE
RECKLESS IMPRUDENCE RESULTING IN PHYSICAL INJURY AND DAMAGE TO PROPERTY
On January 21, 2016 at about 8:40 in the evening, a vehicular accident transpired along Maharlika Highway, Brgy. Caalibangbangan, Cabanatuan City involving:
Vehicle 1 – Racal TS 125 motorcycle, color black, no license plate attached (for registration), owned and driven by LENARD BARCENA y CORPUZ, 20 years old, single, holder of student
permit with number CO5-15-808496, and a resident of Purok Juan Luna, Brgy. Pinagpanaan, Talavera, Nueva Ecija; and
Vehicle 2 – Metro TVS motorcycle, color black, bearing license plate number 1279-NZ, driven by RONEL AISPORNA y DELA CRUZ, 17 years old, no driver’s license presented, and a
resident of C.M. Recto, Brgy. Pinagpanaan, Talavera, Nueva Ecija.
Initial investigation conducted disclosed that vehicle 1 was travelling towards north direction using the outer lane of Maharlika Highway going to Talavera, Nueva Ecija, while vehicle 2
was running on the opposite direction going to City Proper of Cabanatuan. Upon reaching the place of occurrence, vehicle 2 executed overtaking to an unknown type of truck and
occupied the opposite outer lane when it accidentally hit and bumped the incoming vehicle 1. As a result, all vehicles incurred respective damages with un-estimated amount.
Likewise, both drivers, vehicle 1 backrider identified as Marisa Barcena y Corpuz, 18 years old, and a resident of Purok Juan Luna, Brgy. Pinagpanaan, Talavera, Nueva Ecija, sustained
injuries and brought to PJG Hospital, Cabanatuan City for medical treatment. The vehicles involved were brought to this office by members of this Station for investigation and proper
disposition.
NARATIVE
RECKLESS IMPRUDENCE RESULTING IN HOMICIDE
On December 22, 2016, at about 9:00 o’clock in the evening, a vehicular accident transpired along Maharlika Highway, in Brgy. Caalibangbangan, Cabanatuan City; involving:
Vehicle 1 - white Isuzu Elf Closed Van, bearing with license plate number UWN-512, driven by JOVERT BASE y DELOS SANTOS, 23 years old, single, driver, resident of Prk. Kamagong,
Brgy. Pamaldan, Cabanatuan City, holder of Professional Driver’s License with ticket number C05-13-009402; and
Vehicle 2 - black Suzuki Smash, bearing with license plate number 5865-IV, driven by FELIX BENEDICTO y DIZON, 46 years old, married, resident of #20, Prk. #1, Brgy. Umangan, Aliaga,
Nueva Ecija, no Driver’s License presented.
Initial investigation conducted disclosed that both vehicles were running towards South direction, Veh1 heading to Carmen-Caban
atuan Road going to Brgy. Pamaldan, Cabanatuan City, wherein Veh1 is running ahead to Veh2, upon reaching the place of occurrence at the intersection part of the said highway at
infront of BL Motors, Veh2 overtook to Veh1 using the right side portion of the road at that juncture Veh1 accidentally hit Veh2. As a result Veh2 loosed control of the steering handle
and hit the road pavement. Both vehicles incurred damages, all of still unestimated amount. Veh2 driver sustained severe injuries and brought to M.V Gallego hospital for medical
treatment. However, Veh2 driver was pronounced dead on arrival by the attending Physician.
Parties involved to this incident together with the branch manager and the spouse of the victim made a settlement.

For your information.
Upon confrontation made at this office, parties involved in this incident agreed that the vehicle 1 driver together with the branch manager MR./ RALPH SERRA promised to give
Php.25,000.00 for the initial payment that they agreed, will be given later around 10:30 AM this date, anf they will return to this office tomorrow, December 24, 2016 at around 8:00
AM to finalize their agreement. Also, the vehicle 1 will be temporarily impounded to this office until this incident is amicably settled.

RECKLESS IMPRUDENCE RESULTING IN PHYSICAL INJURY AND DAMAGE TO PROPERTY (HIT AND RUN)
On August 19, 2016 at about 7:30 in the evening, the two (2) consecutive incidents of Hit and Run perpetrated by (V4) colored red Owner Type Jeep lowered driven by unidentified
male person transpired along Maharlika Highway in Brgy. Caalibangbangan and Brgy. Mayapyap Sur, Cabanatuan City. Other involved were the following vehicles:

Vehicle 1 - Motorized Tricycle (Kolong-kolong) Motorstar, colored blue bearing Plate number NE-85753, driven by KIER CARLO DELA CRUZ Y GATAB, 24 years old, single and a resident
of Sitio Subdivision, Brgy. Caalibangbangan, Cabanatuan City;
Vehicle 2 - Motorcycle Honda XRM, colored red, bearing Plate number 4622-SM, driven by ANGELO DE OCAMPO Y PARAISO, 25 years old, single and a resident of Purok 2, Brgy.
Maturanok, Guimba, Nueva Ecija; and
Vehicle 3 - Motorcycle Suzuki Smash, colored red, bearing Plate number CC-93034, driven by PATRICK SANTOS Y PATRICIO, 18 years old, college student and a resident of Brgy. San
Juan Bliss, Pantabangan, Nueva Ecija.
Initial investigation conducted disclosed that, V1 traveling towards South direction, along Maharlika Highway, going to City proper of Cabanatuan, while V4 heading towards in same
direction behind the former. Upon reaching Brgy. Caalibangbangan, Cabanatuan City, V1 executed turning left with proper signal lights when accidentally hit by the latter, but ignored
it and ran away from the scene. After a few meters away, the fleeing vehicle (V4) was then again committed incident when it accidentally hit from behind the V2 and V3 who were
also traversing the road of same direction in Brgy. Mayapyap Sur, this City, and then same fled towards to undisclosed direction after the incidents. As a result thereof, all vehicles
involved incurred respective damages with unestimated amount. Likewise, all drivers, backride of V2 identified as JERICK ARELLANO Y PARAISO, 18 years old, a resident of Purok 2,
Brgy. Maturanok, Guimba, Nueva Ecija and backrides of V3 identified as JEROME VILLANUEVA Y VALMONTE, 21 years old, single and JAYVEE DE LEON Y GAGARIN, 17 years old, both
residing at Brgy. San Juan Bliss, Pantabangan, Nueva Ecija sustained injuries were brought to PJG Hospital, this City by Rescue Team of Cabanatuan City for medical treatment.

PROGRESS REPORT
This pertains to the reported incidents of Hit and Run transpired in two (2) nearby places: Brgy. Caalibangbangan and Brgy. Caalibangbangan and Brgy. Mayapyap Sur, Cabanatuan City
on August 19, 2016 at around 7:30 in the evening respectively wherein based on initial information received by this office as speculated by some witnesses, it involved a sole
unidentified suspect driving a red colored lowered owner type jeep (V4) along Maharlika Highway of said places towards City proper (South direction). Other involved were the
following vehicles: (V1) Motorstar kolong-kolong motorized tricycle, clored blue, bearing license plate number NE-85753, driven by KIER CARLO DELA CRUZ y GATAB, 24 years old,
single, and a resident of Sitio Subdivision, Brgy. Caalibangbangan, Cabanatuan City; (V2) motorcycle Honda XRM, color red, bearing license plate number 4622-SM, driven by ANGELO
DE OCAMPO y PARAISO, 25 years old, single, and a resident of Purok 2, Brgy. Maturanok, Guimba, Nueva Ecija, and (V3) Suzuki Smash motorcycle, color red, bearing license plate
number CC-93034, driven by PATRICK SANTOS y PATRICIO, 18 years old, college student and a resident of San Juan Bliss, Pantabangan, Nueva Ecija.
After further investigations conducted by the investigators assigned to this case, it has been established that the said incidents are two (2) seperate cases of vehicular accidents in
different circumstances. Based on the statements of eyewitnesses and antecedent facts recorded in the CCTV installed in Petron Gasoline Station near at the scenes. The incident in
Brgy. Mayapyap Sur was recordedly happened at 7:55 in the evening which complicates to the hit and run incident in Brgy. Caalibangbangan recorded at 8:04 in the evening. This
conveys the fact that V4 was not involved in the earlier incident as above-mentioned.
Another facts gathered upon follow-up investigation were that V2 was traveling towards North direction while V3 was heading towards the opposite direction (South) and upon
reaching the place of occurence, both collided to each other and later found the debris and the two (2) vehicles as well in V3 lane as resulted by the collision, anticipating that V2 had
occupied the opposite lane taht possibly caused the incident. However, result of medical examination shows that the driver of V3 was under the influence of liquor at the time of
incident.
Continious follow-up investigation is still going to gather data for the possible identification of the fleeing driver to file charges against him in response to the plea of the victim (V1).

NARATIVE
On April 9, 2016 at about 3:10 in the afternoon, a vehicular accident transpired along Maharlika Highway in Brgy. Caalibangbangan, Cabanatuan City involving:
Vehicle 1 – color black Suzuki Raider, bearing license plate number BD-4671, owned and driven by MICHAEL JAY DELAS ALAS y ANGOR, 25 years old, single, and a resident of
Homestead, Talavera, Nueva Ecija, holder of a professional driver’s license with number CO5-15010614; and
Vehicle 2 – Kawasaki Bajaj motorized tricycle, black in color, without plate number displayed by NOLITO ARENAS y MACTAL, 50 years old, married, resident of Purok 5, Brgy.
Dalampang, Cabanatuan City, no OR/CR and driver’s license presented.
Initial investigation disclosed that vehicle 1 was running heading to South direction while vehicle 2 on the opposite direction. Upon reaching the place of accident, the vehicle 2
executed left turn occupying the lane of vehicle 1. At that juncture, the incoming vehicle 1 accidentally collided to vehicle 2. Due to the strong impact, both drivers and the back rider
of vehicle 1 identified as KEVIN MABILIN y SARTE, 24 years old, single, and a resident of Homestead, Talavera, Nueva Ecija, sustained injuries and were all brought to a nearby hospital
for medical treatment while their respective vehicles incurred unestimated amount of damages.

Initial investigation conducted disclosed that vehicle 1 was traveling towards North direction, along Maharlika Highway bound to Talavera, Nueva Ecija, while vehicle 2 was heading
towards opposite direction (South), going to City proper of Cabanatuan City. Upon reaching the curved road in aforesaid place, vehicle 2 was occupied the opposite lane when
accidentally hit the incoming vehicle 1. As a result, vehicles involved incurred heavily damages with unestimated amount. Likewise, both drivers sustained injuries and were brought
to MV Gallego Hospital, this City for medical treatment and AB Test. the vehicle 1 was brought to this office for further investigation and proper disposition, while the vehicle 2 still at
the scene due to unavailability of towing services capable of getting the said truck.

On May 15 , 2016, at about 8 :10 in the morning, a vehicular accident transpired along Maharlika Highway, infront of BL Motors in Brgy Caalibangbangan, Cabanatuan City, involving
(Veh1) Tamaraw FX, dark blue in color plate number SEP-881, registered to Bureau of Internal Revenue, driven by FLORENCIO SEVILLA y SAMSON, 52 years old, married, PNP member,
resident of Block 34, Lot 06, AGL Height ll, Brgy Caalibangbangan, Cabanatuan City, no Driver’s License presented and (Veh2) Yamaha Mio, black in color, plate number NC-88757,
registered to Cris De Vera, driven by RONALD GREGORIO y HULAR, 20 years old, single, resident of P-Gregorio Street, Brgy Lingunan, Valenzuela City, holder of Non-Professional

Initial investigation disclosed that Veh-1 is running in Maharlika highway heading towards South direction (to Manila) while the Veh-2 is running in the opposite direction (to Talavera),
upon reaching the curve diversion road thereat, Veh-2 overtook another vehicle then accidentally crashed to the incoming Veh-2. Due to the strong impact, driver of Veh-2 sustained
injuries was rushed to premiere hospital for medical treatment while his back rider identified as ANGELICA CABILDO y GUILLERMO, 19 years old, single, resident of #455 Kaingin
Street, Brgy Parada, Valenzuela City who also sustained injuries was rushed PJG hospital for medical treatment. Both vehicles incurred unestimated amount of damages, Veh-1 on its
front bumper and adjacent parts while Veh-2 on its front wheel head light and adjacent parts. However, the debris was seen in Veh-1 lane.

After confrontation at this office by the driver of vehicle 1 and the father of vehicle 2 driver identified as DAMASO DERONIA GREGORIO of Valenzuela City, they agreed that the repair
on the damages done on vehicles involved during the accident will be shouldered by their own expenses and with regards to the medication of vehicle 2 driver, they made an
agreement that they will return to this office after verification of the amount of medical expenses of vehicle 2 driver to continue conference for possible settlement of this case. In this
regard, parties agreed for the temporary settlement of this case.

On April 21, 2017, at about 3:30 in the morning, a vehicular accident transpired along Maharlika Highway in Brgy. Caalibangbangan, Cabanatuan City involving:
Vehicle 1 - blue Euro motorcycle, no license plate number displayed, driven by ALBERTO ALONZO y PESTAÑO, 54 years old, married, resident of #273, Brgy. San Mariano, Sta. Rosa,

Initial investigation conducted disclosed that Veh1 is traversing towards North direction and Veh2 is traversing towards opposite direction, upon reaching the place of occurrence,
Veh1 occupied the lane of Veh2, at that juncture, Veh1 accidentally sideswiped Veh2.
As a result, Veh1 loosed control of the steering handle and hit the road pavement. Both vehicles incurred damages, all of still unestimated amount. The driver of Veh1 sustained
injuries and brought to PJGMRMC hospital for medical treatment. Both parties involved to this incident agreed to settled this incident amicably.

Cauayan, Isabela and driven by RONNIE PACINOS y HERNANDEZ, 49 years old, married, driver, holder of Temporary Professional Driver’s License with number N04-91-205767 and a
resident of Liwan Norte, Enrile, Cagayan; and
Vehicle 2 - Motorized Tricycle Kawasaki Bajaj CT100, colored red, bearing Plate number 4517-RJ, owned and driven by MARIO DELA CRUZ y SANTOS, 57 years old, married, helper, no
driver’s license and a resident of Purok Sampaguita, Brgy. San Miguel na Munti, Talavera, Nueva Ecija, believed to be under the influence of liquor.
Initial investigation conducted upon interview with both drivers disclosed that, V1 was traveling towards South direction along Maharlika Highway going to City proper of Cabanatuan
and upon reaching the aforesaid place of occurrence, when V2 which came from the opposite road shoulder facing North direction was suddenly executed U-turn and entered to the
said Highway accidentally collided to each other. As a result thereof, V1 incurred damage on its front left fender and also V2 on its sidecar wheel with unestimated and were brought
to this Station by PCP 2 for further investigation and proper disposition.
However, upon confrontation made to this Office by parties concerned they agreed for the settlement of this case.

On March 9, 2017 at about 10:30 in the morning, a Non-Motor Vehicle Traffic Accident transpired along Maharlika Highway in Brgy Caalibangbangan, Cabanatuan City wherein one
Isuzu ten wheeler truck, green, plate number RGN-971, registered to Sterling Central Houseware Trading, driven by RICARDO VALDEZ y DOMINGO, 44 years old, married, resident of
Brgy Loag San Agustin, Isabela, holder of Professional Driver’s License with number B12-13-000815 accidentally hit a pedestrian identified as CARLOS VERA CRUZ y PUBLICO, 46 years
old, married, resident of Brgy General Luna, Sto Domingo, Nueva Ecija.
Initial investigation disclosed that the Isuzu ten wheeler truck was travelling heading to South direction ( to Manila) while the pedestrian was then walking on the right side shoulder
of the said truck and about to crossed. At that juncture, the isuzu truck accidentally hit the pedestrian. As a result, the pedestrian who sustained injuries was brought to PJG hospital
for medical treatment.
