This Office announces the acceptance of application for Teacher I positions in the
Kindergarten, Elementary and Junior High School levels for School Year 2018-2019 (Batch 2)
starting August 13, 2018 until August 20, 2018 (school level).

Hiring guidelines shall be based on DepED Order No. 7, s. 2015, titled, Hiring Guidelines
for Teacher / Positions for School Year 2015-2016.

Attention is invited to item 5.2 of DepED Order No. 7, s. 2015: Application Process &
Procedures — An Applicant shall submit to the head of elementary or secondary school where
a teacher shortage or vacancy (regular and / or natural) exists, a written application with
the Applicant Number indicated, supported by the following documents.

CSC Form 212 (revised 2017) in two copies with the latest passport size ID picture

Il Omnibus Certification of authenticity and veracity of all documents submitted signed

by the applicant

EDUCATION
a) Certified copy of transcript of records (TOR)
Baccalaureate Degree
Certificate in Professional Education (18 units), if applicable
Masteral or Doctoral Degree, if applicable

b) Certification of General Weighted Average (GWA) in baccalaureate degree

c) Certification of General Weighted Average (GWA) of the eighteen (18) professional
units in education (if applicable)

IV. TEACHING EXPERIENCE

a) Copies of service record / certificate of employment, performance rating and
school clearance for those with teaching experience

v. ELIGIBILITY
a) Certified photocopy of PRC professional identification card or a PRC
certification showing the teacher's name, LET rating and other information
recorded in the PRC Office
b) Certified photocopy of ratings obtained in the Licensure Examination for
Teachers (LET) / Professional Board Examination for Teachers (PBET)

VI SPECIALIZED TRAINING AND SKILLS

a) Certificates of specialized training, if any

OTHER REQUIREMENTS
a) Certified copy of Voter's ID and / or any proof of residency as deemed
acceptable by the School Screening Committee
b) NBI Clearance
c) Drug Test
d) Psychological Test administered by reputable / recognized institution
e) Neuro-Psychiatric Test administered by reputable / recognized institution

Vill CLEARANCE (if the applicant is from Private Schools)

a) Certificate of Clearance duly notarized (please see attached template)

All documents must be compiled in a legal sized folder, properly labeled, with table of
contents and pagination.

Original copies of the above documents shall be presented to the School Screening
Committee who is tasked to verify and certify as to completeness, veracity, accuracy and
authenticity of documents. The school head shall then sign under the phrase "VERIFIED AND
CERTIFIED CORRECT"

All teacher-applicants shall take the English Proficiency Test on September 01, 2018
(tentative) at Casimiro A. Ynares Sr. MNHS, Taytay, Rizal. Teacher-applicants are required to
register online at http://deped.in/EPT2018B2SDORlZAL on or before August 22, 2018.
Applicants who already took the EPT in 2016 & 2017 need not take again and shall submit to
the Division HRMO the result of their EPT duly certified by the Division where they took the
test.
Submission of pertinent documents of teacher-applicants per District (with
transmittal) to the Schools Division Office shall be on August 23, 2018 ONLY.

As per item 8.10 of DepED Order No. 7, s. 2015, - The RQA shall be valid for a
period of one (1) school year, hence, all applicants included in RQA SY 2018-2019
(DepED Order No. 22, s. 2007) who were not hired as regular I permanent need not
required to re-apply and undergo selection process.

In the event that schools division office still has available teaching positions after
having implemented DepED Order No. 7, s. 2015, DepED Order No. 22, s. 2015 entitled Hiring
Guidelines for the Remaining Teaching Positions Effective SY 2015-2016 shall be used

 Certificate Clearance
This is to certify that Ms./Mrs./Mr. (complete
name of person applying as teacher at DEPED-Rizal Province) has no outstanding obligations at
(name of school).

In particular, Ms./Mrs./Mr. __________________________________ has resigned

from our school as of _____________________________(specific date) and has faithfully
completed the terms of her/his employment as faculty member including full compliance with
the required period which s/he must serve as part of her/his employment contract at our
school.

This certificate of clearance is being issued upon the request of Ms./Mrs./Mr.
