COMPREHENSIVE NURSING HISTORY

HISTORY OF PRESENT ILLNESS

5 years prior to admission, patient had onset of behavioral changes associated with
aggression and violent gestures that led to hurting family members, and was hyperactive. In
most occasions, patient then, seek out consultation at Zamboanga City Medical Center and was
diagnosed with Brief Psychotic Disorder. Onset of behavioral changes were associated with
sudden mood changes such as excessive laughing or crying. Patient was reported to have
insomnia, poor appetite, excessive crying, and hyperactivity that lead to the diagnosed as
Schizophrenia. Patient was reported to be stable until 9 days prior to consultation, patient was
seen with a recurrence of behavioral changes accompanied with an aggressive behavior,
insomnia, poor appetite, excessive crying and increase in energy. There were no signs of
hallucination and suicidal ideation. Patient was in for consultation after not being able to take
the medications prescribed which led to the persistence and worsening of symptoms.

PAST HISTORY

Patient had a previous history of hypertension. No history of surgeries and allergies.

FAMILY HISTORY

No family history of hypertension, Diabetes Mellitus and Asthma. Mother, father,


aunt (the sister of the patient’s father), and sister is positive for Schizophrenia.
