SUBJECT COMPETENCY ASSESSMENT FORM

ln the interest of the service, and to ensure that the competency needs of PAO
employees specially the non-lawyers are addressed, a new Competency Assessment
Form will be utilized.

The attached form must be fully accomplished by both the employee and their
immediate supervisors and submitted to the Training Section, Field Operations and
Statistics Service, five (5) working days after receipt.

For strict compliance.

Let all concerned be furnished with a copy of this Order for their information and
guidance.
