Dear Parent or Guardian:

We believe there are many educational values to be gained from a selected, out of school experience.
We are planning a field trip, described below, to supplement our classroom program. Please indicate
whether or not your child has your permission to participate, sign and return this completed form to the
school along with a check made payable to District 97 in the amount of: $ 8.00 DUE BY FRIDAY, 2/1.

Description of Field Trip

To: Attend Play: Three Little Pigs, Stahl Family Theater, 5900 West Belmont Avenue, Chicago, IL

Date: Tuesday, February 26, 2019

Student’s Cost: $8.00 (Checks only made payable to School District 97)

Time of Departure: 9:40 am Approximate Time of Return: 12:15

Means of Transportation: Bus

Additional Information: The field trip to see the play “Three Little Pigs” relates to our upcoming WUOS – Adapting and Writing Fairy Tales. In this unit
students will look at various adaptations of fairy tales, which would include the play “Three Little Pigs”. Students will not only see a play about a fairy tale, but they
will also be given the opportunity to write their own fairy tale that is not only a well-crafted tale, but to story-tell their tale with drama, precise action and language that
captures the hearts and minds of the listener.
Common Core Standards covered: Speaking & Listening Standards; Writing Standards; Reading Standards; and Language Standards.
