In connection with the Annual Sports Festival held on September 27-28, 2018, the
undersigned would like to ask for permission that the following student-athletes of these teams
in basketball boys be allowed to continue the over-time game.

These teams are:

Blue Team’s Basketball Boys

Green Team’s Basketball boys

Furthermore, it is requested that they be excused from their classes this afternoon, 3pm
October 3, 2018, and they be allowed to make up for activities and assignments that they may
miss.

Madam:

I have the honor to recommend the designation of ANGELO S. SINFUEGO ,

TEACHER 1 as Health Coordinator for SY 2018-2019.
As a designate Health Coordinator, he/ she is the focal person in the implementation of
the Health and Nutrition Program in schools and is expected to fulfill the following duties and
responsibilities:
 Secure health cards of the learners;
 Assist the School Health and Nutrition Unit during Medical and Dental
Examinations;
 Assist in the implementation of DOH and DepEd collaborative programs;
 Perform first aid treatment;
 Attend meetings, orientations and seminars related to Health and Nutrition
Programs;
 Establish databank of Reports on Health and Nutrition Programs
 Submit Health and Nutrition Program related reports; and
 Monitor and refer incidents and urgent cases to School Health and Nutrition
personnel.

This office believes that she can completely handle his/ her extra assignment as he/she
does have faith in him/herself that she can attend to other school related matters effectively.
