I. General Information
A. National Asset Registry Number: (Leave Blank for ARD use)
B. Organization Code: Agencies Code in UACS format. Should be the
agency code of the agency booking the property.
- Department Code (2 digit), Agency Code (3 digit), Operating Unit
(2 digit), Lower Level Operating Unit (5 digit)
i.e DepEd, Office of the Secretary, Region, NCR
-DepEd- NCR, Valenzuela - “070010813016”
C. School Identification Number: Per DepEd identification system
D. Property Type: As per General Accounting Manual (GAM) account
code.
i.e. 10604010-00 - building, 1060402-000 - school building,
land -10601010-00
E. Asset name: proper/common name - i.e. A Deato Elem. School Bldg 2
F. Property Number/ Identification code: property number of the asset
in the RCPPE or per DepEd format
-Region-Division-School abbreviation – Bldg No.
i.e. NCR-PSY-ADES-Bldg1 where:
NCR - National Capital Region
PSY - Division of Pasay City
ADES - Agapito Diaz Elementary School
Bldg1 - Building Number

G. Container Property Number: Property number of land/lot in which a


building or several buildings are compounded.
H. Acquisition Date: Date of acceptance or date when the responsibility of
the property is officially transferred to the Agency.
I. Acquisition Mode: Per DepEd defined mode of acquisition
i. Purchased – Asset was procured or bought by DepEd
ii. Constructed – Asset was constructed by DepEd
iii. Transfer – Asset was constructed by private entity/ Government
Agency or International Organization (i.e. by DPWH, WB, JICA)
iv. Donation – Asset was received from private entity or LGU
J. Reference Number: documentary reference number (CTC number, TC
number, OR, CR, JEV No.).
K. Serial Number: (no need to fill up)
L. Remarks: Any additional information, notification on status of property
i.e. property was never appraised etc.

M. Location - will be used to locate the asset

N. Legal Information
a. Owner: legal owner, not all government land and buildings being
used are owned by the government but is essential to government
operations.
b. Legal impediments: Comment any legal problems with the
property (under litigation, with on-going case with case #... etc).
c. Transferred From: Information of the original owner.
d. Transferred To: Recipient Information.
e. Date of Transfer: Date of transfer from the original owner to the
recipient i.e. Donation from owner to the Agency.
II. Financial Information – For financial information, if value presented is not
the current year, state the year of the valuation. (From Accounting Office)

A. Acquisition Cost: Amount of the acquisition cost of the asset.


B. Book Value: Amount recorded as book value of the asset.
C. Depreciation: Refers the accumulated depreciation.
D. Asset Life: Refers to the estimated number of years of which the asset
will be useful or the asset lifespan.

E. Number of years use: Refers to the number of years the asset has been
used/utilized.
F. Remaining Life: Asset Life less number of years used
G. Market Value/ Sound Value: Refers to the market value of the land, or
sound value for buildings/ school buildings. It is the estimated worth of
an asset that is based on how much a buyer would be willing to pay the
seller in an arm’s length transaction.
H. Assessed Value: Refers to the value placed on taxable property by the
assessor for ad valorem tax purposes, synonymous to taxable value
I. Appraised Value: Refers to the assigned value of the appraiser
J. Date of Appraisal: Refers to the date the asset appraised
K. Improvements: Refers to the capital improvements made to the asset
extending the use or life.
L. Date of Improvement: Refers to the date of which the asset is improved
M. Mode of Disposal: -Do not fill up-
N. Disposal Value: -Do not fill up-
O. Currency: -Do not fill up-
P. Replacement Value: (can be the amount insured or book value if no
value available, is the cost of replacing/ reconstructing a structure)
i. Asset: for structure (building/ school building)
ii. Contents: owned by the DepEd, contained inside the structure
iii. Total: Total amount of the asset plus contents.
III. Insurance Details - This information may be provided by the office who
keeps the insurance policy, if any. Please note that all the information
needed here can be found in the insurance policy. Further, no need to fill up
insurance information for land property type of asset. For building and
school sites, if insured, fill up the sum insured information, if not, provide
only sum insurable information.
A. Sum Insurable (In cases asset is not insured)
i. Asset: Book Value or value requested to be covered by insurance
ii. Contents: Book Value or value requested to be covered by insurance
Total: Total Amount Value of the asset plus the content.
B. Insurer Name or ID: Name of the Insurer (GSIS or private insurer)
C. Issuing Branch: Branch office of insurer

D. Insured Name: Name of Agency
E. Policy Number: Insurance number provided by insurer
F. Type of Policy/Coverage: Perils covered by the insurance (Item Perils)
G. Period: Validity (to – from) of insurance
H. Underwriting year: Year policy was issued
I. Currency: Currency of insurance
M. Sum Insured - (for insured assets, per item entry
Cat – catastrophic; non-cat – non-catastrophic coverage)
i. Asset:
-Cat: Amount of coverage for catastrophic events e.g. earthquake,
floods etc.
