We coordinate with our local partners to develop personalized rehabilitation plans
in an effort to increase mobility and independence of our residents. These partners
from local rehabs consist of occupational therapists, physical therapists, and

strategy to help you or a loved one reach your goals through targeted therapy.

 Level of Care
DEFINITIONS
Level of Care: Care rates are based upon each individual resident’s required assistance with daily
living including but not limited to: Emergency response system, trained caregivers, and daily
room service. The care level is determined by our Resident Care Director through a personal
assessment prior to the resident’s move in date and as necessary thereafter.

Level 1:
uu Three well-balanced meals will be provided in dining Care Level Rates
room and or delivered to room with clean up.
uu Assistance with oral medication administration by Assisted Living
trained medication technicians. Level 1 $500
