TEST REQUEST FORM

SERVICE CONDITIONS
1. Customer shall fill in the parts of the test request form; ‘’client information’’, ‘’sample description’’, and ‘’tests’’.Test reports
are prepared according to supplied information. Customer is responsible to the correctness, the efficiency, the entirety and
the currency of the information in the request form. If a new report is asked to be prepared due to wrong or inadequate
information minimum reporting price will be charged. Firm seal part should be sealed and sent to Ekoteks by the customer.
Firm seal which appears on test request form means the approval of customer.
2. The customer data (information about the company, sample, test results and test reports) shall not given to third party
without a written permission by the customer. If there is a buyer on the request form, test report and results are sent to
buyer or/and agency without approval of customer by Ekoteks.
3. The acceptance date of the sample is recorded as the date of completion for samples which are not processed due to
missing information or insufficient sample. These samples are kept max 1 week in the laboratory. At the end of this time (1
week), if the customer doesn’t accomplish the deficiency, customer can not assert a claim regarding the sample.
4. Immediately starting testing process the Customer Application Form which includes the requires fields ( date-out, report
number, test standards and total cost)will be faxed to the customer.
5. Any cancellation request must be declared in 24 hours for normal service, 4 hours for express service after the fax message.
Cancellation will be not accepted for shuttle and urgent service and the tests which are already done will be invoiced.
6. The customer is liable to send the samples to the laboratory and the laboratory is liable to send the test reports or the
tested samples to the customer.
7. The information about the amount of samples will be received from customer relations department.
8. Retained sample and tested dimensional stability, appearance samples are kept for 3 months. The laboratory will not repeat
the test with the same samples for any complaint which was declared more than 3 months ago. Tested sample is given with
written approval of customer. One copy of test request form, a piece of sample, test results and test report will be kept for 5
years.
9. Regular service period is 3 working days, express service period is 2 working days, shuttle service period is 1 working day
and urgent service is given in same day. If the sample is received until 14:00 am, the day which the samples received in will
be served as the first working day. The suitability of the requested tests for the service period will be approved from the
customer relations department.
10. The fee of the service and the tests will be paid cash by the customer. Total amount of payment is calculated over the
exchange rate on American Dollars banknotes of the Central Bank of the republic of Turkey on the date of the invoice. In
case of the customer delays or fails to pay, Ekoteks has the right not to prepare a new test report until the payment has
paid.
11. If the customer request a test which is cheaper than 10 USD, the test invoiced as the minimum report price 10 USD. If the
customer require revise report (cancel/ add/ amend information on issued report). It will be priced as 5 USD.
12. The final revised Ekoteks Price List will be effective if there is no agreement with different circumstances between the
parties. Ekoteks has the right to revise the price list. The price of preparing reports includes only reporting in one language.
The final revised Ekoteks Price List is not included V.A.T.
13. Ekoteks Laboratory is obliged to exercise accurate and careful execution of the work, undertaken by this agreement. If there
are deficiency and errors in any of the laboratory reports and in case it is determined that these are caused by Ekoteks or its
employees,the liability of Ekoteks shall be limited to the test fee without any interest. Ekoteks shall not be held liable to
indemnify the damages and collaterals incurred by the Customer or by third parties arising from deficiencies or defects in
the test report.
14. In case of any dispute, an arbitrator committee appointed by General Secretariat of İstanbul Textile and Apparel Exporters
Associations (İTKİB) and/or İstanbul Court of Justice will be authorized.
15. Ekoteks Laboratory shall not be liable for any loss or damage resulting from any delay or failure in performance of its
obligations hereunder resulting directly or indirectly from any cause that is beyond the control of the company; natural
disaster, war, terror, government restrictions, law, strike, lock-out, machine failure, fire, accident.If any such event occurs,the
company may immediately cancel or suspend its performance hereunder without incurring any liability whatsoever to client
